Total laparoscopic hysterectomy (TLH) in the presence of patients with previous cesarean section (CS) is becoming increasingly common. When performing TLH in these patients, bladder adhesions to the uterus may make dissection much more difficult with higher complication rates. The aim of this study was to assess the safety of TLH in patients with previous CS in an OBGYN residence program. Retrospective study of all TLH performed at our center for either benign or malignant conditions. Of our study cohort, 40 % had undergone one or more previous CS. Average surgical time was 128 min for patients without previous CS and 136 min for patients with previous CS (p = NS). Conversion to laparotomy was required in 1 % of cases showing no variation between the CS and non-CS groups. The overall complication rate among patients undergoing TLH was 3.5 %. Major complication rate was of 3 % (n=14), 5 cases with previous CS and 9 cases with no previous CS (p = NS). Urologic lesion was the most common major complication, accounting for 1.5 % (n=7) of all cases, 3 cases with previous CS and 4 with no previous CS (p = NS). Of urologic complications, three were cystotomies, 1 with no previous CS and 2 with previous CS (p = NS). TLH in patients with 1 or more previous CS is technically feasible. In the hands of thoroughly trained laparoscopic surgeons using a standardized technique, it is a safe procedure with minimal complication rates and may be even performed by OBGYN residents ensuring the same success rates.
Introducción y objetivos: El embarazo ectópico es una condición potencialmente mortal, con una incidencia del 1 al 2%. El 97% se produce en las tubas uterinas, y el 80% de éstos se encuentran en la región ampular. El objetivo de esta revisión es exponer una actualización del enfrentamiento y manejo del embarazo ectópico tubario. Métodos: Mediante el uso de la base de datos Epistemonikos, Scielo, Cochrane y Pubmed, se revisó la literatura existente sobre embarazo ectópico tubario. Resultados: El diagnóstico de embarazo ectópico tubario implica una combinación de síntomas clínicos, serología y ultrasonido. El manejo médico es una opción segura y efectiva en la mayoría de las pacientes hemodinámicamente estables. En caso de fracaso de tratamiento médico, paciente incapaz de mantener seguimiento, embarazo ectópico roto o embarazo heterotópico, debe ser manejado con tratamiento quirúrgico idealmente por laparoscopía. Independiente del tratamiento utilizado, existe gran probabilidad de éxito y escasas complicaciones. Conclusiones: Esta actualización describe la incidencia, factores de riesgo, diagnóstico, y manejo del embarazo ectópico tubario. Es importante un diagnóstico y tratamiento temprano, para reducir complicaciones asociadas a esta patología.
Introducción: La Simulación clínica es una herramienta de aprendizaje complementaria a los métodos tradicionales en la práctica clínica. Permite desarrollar habilidades y destrezas en escenarios seguros. La División de Ginecología y Obstetricia de la Universidad Católica (UC) imparte un curso de emergencias obstétricas para especialistas utilizando esta metodología. Presentamos la evaluación de los especialistas que realizaron el curso en el año 2016. Materiales y métodos: se realizó un curso de simulación de escenarios de emergencia en obstetricia, con un total de 16 horas separadas en 2 días. Con simuladores de alta y mediana complejidad. Tutoriados por instructores acreditados por el Institute of Medical Simulation de la Universidad de Harvard. Al finalizar el curso los participantes respondieron una encuesta escrita de diversos tópicos del curso. Resultados: participaron 93 especialistas. 76% de los participantes estuvo completamente de acuerdo y 23% de acuerdo con que la práctica con modelos simulados mejora la destreza relacionada con la realización de los procedimientos propuestos en el curso. El 100% de ellos indica que recibió retroalimentación considerada como útil durante las sesiones de entrenamiento y consideran que están completamente de acuerdo o de acuerdo con que esta metodología les permite cometer errores en forma segura. Conclusiones: la implementación de la metodología de simulación clínica en la docencia de emergencias obstétricas es muy bien evaluada por los especialistas, ya que entrega retroalimentación académica útil de cada caso y les permite cometer errores sin riesgos para los pacientes con un alto nivel de realismo.
Introducción: El creciente aumento de los alumnos de pregrado, asociado a la disminución en el número de partos, han reducido la exposición de los alumnos a procedimientos como la episiotomía. En este proyecto se implementó un modelo docente utilizando fantomas de bajo costo para el entrenamiento en la reparación de episiotomía. Materiales y Métodos: La instrucción fue realizada a 106 internos de sexto año en grupos de 5 a 10 alumnos y 12 residentes de primer año de la especialidad. Se revisaron los aspectos teóricos relacionados a esta técnica quirúrgica. Cada alumno tenía a su disposión dos fantomas de alta densidad. Los materiales de sutura y el instrumental eran similares a los usados en la práctica clínica. Se realizaron 2 evaluaciones a los alumnos: un minitest teórico y una evaluación de la actividad práctica. Resultados: El resultado promedio de la evaluación teórica fue de 6,3 para los internos de 6º año y de 6,7 para los residentes de primer año de Especialidad. En los aspectos formativos de la actividad práctica, siendo la evaluación máxima 4, los alumnos calificaron la actividad con un 3,66 . En términos de satisfacción de los alumnos con la actividad, sobre una evaluación máxima de 5, los alumnos entregaron una calificación de 4,7. Finalmente, el promedio de los alumnos calificó la actividad muy cerca de lo excelente. Discusión: La implementación de fantomas de espuma de alta densidad, facilita el acceso a modelos de reparación quirúrgica más económicos y permite repetir el ejercicio hasta alcanzar los objetivos establecidos.
