Este trabajo tiene como propósito promover formalmente el movimiento de seguridad del paciente y mejorar la calidad en el Perú con una agenda visible y metas que estén al alcance de las instituciones de salud. Esta publicación esta destinada a abordar temas poco conocidos en relación a la seguridad del paciente, mejora continua de la calidad, gestión de riesgos y medicina basada en la evidencia. El desarrollo de estas habilidades es fundamental para que los administradores de salud y profesionales clínicos del Perú puedan dirigir organizaciones capaces de lograr resultados de alta calidad. La serie de artículos ofrecerá conocimientos y guías basados en trabajos fundamentados y en el conocimiento contemporáneo adaptado a la realidad de economías en desarrollo como la peruana.
El Instituto de Medicina de los Estados Unidos, IOM (2000, p. 16), incluye tres aspectos en la definición de la calidad: "La seguridad contra lesiones accidentales, la prestación del servicio de forma consistente con el conocimiento médico actual y las mejores prácticas, y la capacidad de cumplir con los valores y expectativas de los consumidores”. La combinación de la teoría de la calidad y la teoría de la gestión conduce a la estructura asociada con la eficacia organizacional (Cole & Scott, 2000; Dean & Bowen, 1994; Leape, 1994; Schimmel, 1964).

Ya en el año 400 A.C., Hipócrates reconoció los posibles efectos perjudiciales que surgen a través de las acciones bien intencionadas de los curanderos. Si bien el origen de la frase es desconocido (Carroll, 2004), el legendario aforismo hipocrático primum non nocere, “primero no hacer daño” (Veatch,1989) es un principio de salud importante y duradero. La palabra iatrogenia, derivada del griego, por lo general se refiere al producto de la atención prestada a los pacientes por los curanderos. Iatrogenia significa literalmente, "producido por el sanador" (iatros significa “curandero” y génesis significa “dado a luz por”).
