17Beta-estradiol reduces the degree of pacing-induced myocardial ischemia in postmenopausal patients with coronary artery disease. The reduction of pacing-induced coronary sinus pH shift is consistent with an anti-ischemic effect of the hormone and is not due to preconditioning, as evidenced by the absence of improvement after placebo.
Los pacientes con angina de pecho, evidencia de isquemia miocárdica y angiografía con coronarias no obstructivas, implican un verdadero reto diagnóstico. Estudios recientes han demostrado que, en esta entidad clínica, el pronóstico no es tan benigno como se pensaba. El síndrome X cardíaco, actualmente más conocido como isquemia miocárdica y arterias coronarias no obstructivas (INOCA: Ischemia and No Obstructive Coronary Arteries), se ha enfocado predominantemente en la disfunción microvascular, sin embargo, los otros mecanismos causales como la enfermedad aterosclerótica oculta, el espasmo de coronarias epicárdicas y el síndrome del corazón sensible, siempre deben ser considerados. Esta revisión basada en la evidencia de los últimos 20 años, plantea una visión práctica del síndrome INOCA. Se aborda la inquietud y esfuerzos de los diferentes investigadores que estudian este síndrome, con la finalidad de lograr un protocolo diagnóstico adecuado, para así orientar mejor la terapéutica según el mecanismo fisiopatológico predominante.
Fundamento: O grau de fluxo TIMI e a contagem quadro a quadro TIMI corrigida (CTFC) são métodos amplamente utilizados para avaliar o fluxo sanguíneo coronariano angiográfico. A medição do fluxo sanguíneo coronariano (FSC) na coronariografia (CAG) padrão despertou grande interesse recentemente, tentando combinar o conceito de CTFC com novos métodos para pós-angioplastia e avaliação da síndrome cardíaca X. Além disso, o fluxo coronariano lento é considerado um critério importante para a angina microvascular. Objetivo: Explorar uma nova abordagem de medição angiográfica quantitativa do FSC com base na detecção densitométrica de contraste na CAG offline, usando um software acessível para obter uma avaliação mais precisa e confiável do FSC. Métodos: Trinta pacientes foram estudados e divididos em 2 grupos: fluxo sanguíneo coronariano normal (FN) e fluxo sanguíneo coronariano lento (FL), de acordo com a definição da CTFC. O MD foi aplicado à amostra do estudo para diferenciar entre FN e FL. A estatística não paramétrica foi usada para avaliar diferenças entre os grupos com p<0,05. Resultados: O valor de referência normal do MD obtido para o fluxo sanguíneo coronariano foi de 9 [5-10] quadros.
