Introdução/Objetivo O Acinetobacter baumanii resistente ao carbapenem, ou CRAB, é uma bactéria resistente a medicamentos que se espalha em hospitais, principalmente em unidades de terapia intensiva. Pode causar pneumonia, bem como infecções em feridas, sangue e trato urinário, de acordo com o CDC. Essa bactéria representa uma ameaça para pacientes hospitalizados porque podem sobreviver em superfícies por muito tempo, podendo ser responsáveis por surtos. O principal objetivo do estudo foi a descrição de surto em hospital referência para tratamento de Covid-19 no Nordeste/Brasil. Métodos Monitoramento do perfil microbiológico durante o período de pandemia pela Covid-19 em 2021, com reconhecimento de principais meses de detecção do Acinetobacter spp. e medidas de controle de surto, em hospital privado com 250 leitos sendo 60 de UTI. Resultados No período da pandemia 2021 os meses com maior isolamento de Acinetobacter spp. foram de março a junho, sendo os antimicrobianos mais prescritos no hospital foram: Piperacilina/tazobactam (16%), Ceftriaxona (14%) e Meropenem (12%). Os Gram negativos representaram 71% dos patógenos isolados em 2020 e 60% em 2021, notando-se um aumento das infecções por fungos de 13% em 2020 e 24% em 2021 (sendo predomínio de não albicans 45/73). O Acinetobacter spp. foi isolado em hemoculturas (5,6% total positivas), culturas de material de vias aéreas (18,9% total positivas) e urinoculturas (1,4% total positivas). O perfil microbiológico evidenciou sensibilidade de 53% a amicacina, 5,3% a Piperacilina/tazobactam e Meropenem e, 88,9% a Polimixina B por microdiluição. No período de maior número de infecções nosocomiais houve escassez de polimixina o que modificou o tratamento de escolha para o tratamento de Acinetobacter spp., sendo recomemndada polimixina por 7 dias e se necessário prolongar terapia trocada para associação de Tigeciclina dose dobrada, Amicacina e Ampicilina/sulbactam dose triplicada. As medidas de isolamento por coorte dos pacientes infectados e aventais de uso único foram determinantes para contenção do surto. Conclusão Durante a pandemia de 2021, o risco de infecções nosocomiais aumentou e, portanto, a utilização de maior número de antimicrobianos de amplo espectro, permitindo através da pressão seletiva o risco maior de surto por CRAB. O isolamento dos pacientes infectados e uso racional de antimicrobianos são as melhores estratégias para controle de surto por superbactérias.
Introdução/Objetivo A multirresistência bacteriana é particularmente comum em bacilos Gram-negativos (BGN), com importantes consequências clínicas quanto à sua disseminação e opções de tratamento. O objetivo deste estudo foi investigar a tendência de BGN multirresistentes (BGN-MR) em departamentos hospitalares de alto risco, entre 2016-2021, com o intuito de detectar alterações do perfil de sensibilidade que possam ter sido impactadas pela Pandemia por Covid-19. Métodos Este é um estudo observacional retrospectivo realizado em departamentos de uma unidade terciária de saúde no Nordeste/Brasil. MDR foi definido como resistência adquirida a pelo menos um agente em três ou mais categorias de antimicrobianos. Avaliados 5 anos de perfil microbiológico que englobaram os anos de 2020 e 2021 da Pandemia. Resultados Os Gram negativos mais prevalentes da instituição são: E. coli, K. pneumoniae e P. aeruginosa. Em 2021, as hemoculturas positivas (N = 300) evidenciaram: 10,6% de P. aeruginosa, 6,3% de K. pneumoniae e 3,3% de E. coli. Nas amostras de secreção respiratória (N = 328): 36,6% de P. aeruginosa, 12,2% de K. pneumoniae e 18,9% de Acinetobacter baumannii. De 2016 a 2021 observamos, respectivamente, um decréscimo de sensibilidade para E. coli da Ciprofloxacina (50%, 74%, 73,9%, 69,2%, 59,2% e 58%), da Piperacilina/tazobactam (100%, 78%, 97,8%, 96,1%, 100% e 91,7%) e mantida sensibilidade para Ertapenem (100%, 81%, 97,7%, 97,4%, 100% e 98,8%) e Meropenem (100%, 33%, 100%, 98,7%, 100% e 98,8%). Para P. aeruginosa observamos decréscimo da sensibilidade para Amicacina (95%, 83%, 81,8%, 85,1%, 81,8% e 75,4%), Piperacilina/tazobactam (55%, 67%, 65,2%, 68%, 81,2% e 33%), Meropenem (65%, 54%, 69,5%, 55,3%, 75,7% e 37%) e polimixina B (100%, 91%, 100%, 91,7%). Para K. pneumoniae detectamos: decréscimo da sensibilidade para Amicacina (100%, 86%, 95,2%, 76,7%, 90,9% e 81,5%), Piperacilina/tazobactam (55,5%, 41%, 50%, 52%, 63,6% e 11,2%), Meropenem (77,7%, 59%, 95,2%, 63%, 87,8% e 14,2%) e polimixina B (100%, 90%, 90,9%, 76,2%). Conclusão A pandemia pela Covid-19 teve correlação direta com aumento da resistência aos gram negativos mais prevalentes em ambientes hospitalares, particularmente na sensibilidade de P. aeruginosa e K. pneumoniae aos carbapenens e polimixina. O uso racional de antimicrobianos e políticas de controle de utilização dos mesmos são estratégias essenciais para preservar opções para o futuro.
Introdução/Objetivo Patógenos Gram-negativos são responsáveis pela maioria das infecções nosocomiais ou associadas aos cuidados de saúde, principalmente casos de pneumonia associada à ventilação (PAV). Pacientes com fatores predisponentes, como vítimas de queimaduras graves, aqueles com função imunológica reduzida e aqueles internados em unidade de terapia intensiva (UTI), apresentam risco aumentado de infecções. Embora a extensão relatada de coinfecção com patógenos bacterianos em pacientes hospitalizados com COVID-19 varie, P. aeruginosa está entre as espécies mais frequentemente identificadas em tais pacientes, com uma proporção maior em pacientes criticamente enfermos de UTI. Além disso, os pacientes ventilados com COVID-19 podem ter maior risco de desenvolver PAV. Nosso objetivo foi avaliar no período de 3 anos da utilização da Ceftazidima/avibactam o impacto no perfil de sensibilidade aos Gram negativos mais frequentes nas infecções nosocomiais. Métodos Avaliação retrospectiva das culturas positivas com isolamento de Gram negativos multirresistentes entre 2019 e 2021 em unidade de atendimento hospitalar terciário no Nordeste/Brasil. Resultados Foram utilizados em 2020 um total de 581 frascos de Ceftazidima/avibactam e em 2021 esse valor até o momento foi de 1313 frascos. Nos isolados de P. aeruginosa (n = 128) observamos o perfil de sensibilidade decrescente a Ceftazidima/ avibactam entre 2019 e 2021, sendo respectivamente a sensibilidade 100% (n = 7), 68% (n = 25) e 60,4% (n = 96). Nos isolados de K. pneumoniae detectamos o perfil de sensibilidade decrescente a Ceftazidima/ avibactam entre 2019 e 2021, sendo respectivamente a sensibilidade 78,6% (n = 14), 72% (n = 25) e 60,3% (n = 58). Para Serratia sp. Foram isolados amostras positivas em 2020 e 2021 apenas, sendo a sensibilidade respectivamente 100% (n = 4) e 81,8% (n = 11). Conclusão Durante o período de pandemia por Covid-19 e maior utilização de ceftazidima/avibactam em infecções nosocomiais foi evidenciado aumento de resistência para Gram negativos com impacto direto na terapia empírica de patógenos MDR.
Although a rare condition, AAE can occur in SLE, either with deficiency or normal levels of C1 inhibitor. The investigation of AAE is challenging, especially when secondary to a disease with such a variable presentation. Thus, in the diagnostic approach of patients with AAE, one should always consider and look for evidence of autoimmune diseases, especially SLE.
