RESUMOIntrodução: A satisfação com a profissão médica tem sido apontada como um fator essencial para a qualidade assistencial, o bemestar dos doentes e a estabilidade dos sistemas de saúde. Estudos recentes têm vindo a enfatizar um crescente descontentamento dos médicos, principalmente como consequência das alterações das relações laborais. Objetivos: Avaliar a perceção dos médicos de formação específica em Portugal, sobre as expectativas e grau de satisfação com a profissão, especialidade e local de formação; razões da insatisfação e intenção de emigrar.
Hidradenitis suppurativa (HS) is as a chronic, inflammatory and recurrent skin disease of the hair follicle. Prevalence of HS is a matter of debate, with estimate rates as low as 0.00033% and as high as 4.1%. HS prevalence appears significantly higher in women and has a mean age of onset in the early 20s. The primary histopathologic event seems to be a follicular hyperkeratosis with plugging, dilatation and rupture of the hair follicle resulting with subsequent inflammation and formation of abscesses and sinus tracts. The cause of this is likely multifactorial, involving genetic factors, cigarette smoking, mechanical stress, obesity, immune aberrant response and hormonal abnormalities. HS, as a chronic inflammatory dermatosis, is associated with a variety of concomitant and secondary diseases such as metabolic syndrome, inflammatory and rheumatologic diseases, depression and malignancy, and, for this reason HS management often requires a multidisciplinary team.
O aumento do número de doentes sob terapêutica antitrombótica aliado à maior prevalência de neoplasias cutâneas implica uma gestão adequada destes fármacos no período perioperatório. Os autores apresentam e discutem o protocolo orientador da prática clínica no serviço de Dermatologia e Venereologia do Centro Hospitalar Universitário de São João. A suspensão ou manutenção da terapêutica com antiagregantes plaquetários e/ou anticoagulantes deve ser avaliada de forma individual, tendo em conta o risco hemorrágico do doente e da intervenção e o grau de risco trombótico do doente. Nestes doentes, as medidas hemostáticas locais são particularmente pertinentes na gestão da hemorragia no perioperatório e devem ser minuciosas. O Serviço de Imunohemoterapia presta colaboração nas situações de difícil controlo da hemorragia.
