Introducción: el bloqueo PENG ha demostrado ofrecer analgesia efectiva en cirugía de cadera. La analgesia tradicional opioide está asociada con efectos colaterales de alto impacto en la evolución de los resultados postoperatorios. Reportamos la calidad de la analgesia postoperatoria en dos escenarios: anestesia general y anestesia espinal más anestesia regional, mostrando una eficacia y seguridad similar. Material y métodos: se recolectaron 60 pacientes consecutivos: grupo 1, anestesia general más PENG, y grupo 2, anestesia espinal más PENG. Se registraron puntajes de dolor en pasivo y activo en el preoperatorio, en la Unidad de Cuidados Postanestésicos (UCPA), a las seis, 12, 18 y 24 horas. Resultados: el promedio de puntaje de dolor para ambos grupos en el postoperatorio fue de 2.0-1.0 en la escala visual análoga (EVA) a las 24 horas, 81.7% de los pacientes no requirieron de analgesia adicional al esquema analgésico de base, de 18.3% de pacientes que requirió rescate analgésico, éste fue de tipo antiinflamatorio no esteroideo (AINE); 70% de ellos fueron 12 horas posteriores a la colocación del bloqueo PENG. Noventa y cinco por ciento de pacientes estuvo satisfecho con la analgesia postoperatoria. Conclusión: el bloqueo PENG está asociado con un alto índice de éxito analgésico en cirugía de cadera y baja incidencia de complicaciones; puede ser considerada una alternativa segura en el esquema multimodal.
