El lupus eritematoso sistémico (LES) es una enfermedad crónica autoinmune que puede afectar cualquier órgano, las manifestaciones clásicas generalmente consisten en artralgias, mialgias, artritis, fotosensibilidad e insuficiencia renal. Casi la mitad de los pacientes presentan alteraciones cardiacas a lo largo de la enfermedad y el taponamiento cardiaco esta descrito como una de las complicaciones tardías, llegando a ser una forma infrecuente de presentación inicial. Presentamos el caso de un hombre de 32 años que consulta por dolor precordial, disnea y edema de miembros inferiores. Se sospecha síndrome coronario agudo, al ingreso se realiza exploración ultrasonográfica con evidencia de taponamiento cardiaco que motivaron intervenciones emergentes que salvaron la vida del paciente. Los paraclínicos descartaron causas infecciosas y un título alto de ANAS, anti-ADN asociado a variables clínicas llevó a diagnóstico de LES. Se inicia manejo inmunomodulador y es dado de alta al día 16 de estancia
Introducción: la infección de vías urinarias (IVU) constituye un problema clínico común en los hospitales pediátricos, representa una de las enfermedades más frecuentes en la infancia, su diagnóstico se fundamenta en el cultivo de orina, donde el principal patógeno aislado es Escherichia coli. Convencionalmente, la decisión de manejo se basa en cuatro factores: la bacteria y su sensibilidad, la localización de la infección, el estado clínico y la edad del paciente. Sin embargo, este tratamiento debe iniciarse mucho antes de conocer el germen y su perfil de susceptibilidad. La creciente incidencia de patógenos gramnegativos, resistentes a los fármacos antimicrobianos comunes ha llevado a serias dificultades en relación a su tratamiento, dejando a un lado el uso de antibióticos clásicos o de primera línea, ante la sospecha de multirresistencia.Objetivo: realizar revisión temática para identificar conceptos referentes a mecanismosde resistencia de los gérmenes gramnegativos, especie de gérmenes multirresistentes yterapéuticas para IVU multirresistente.Metodología: se efectuó revisión en las bases de datos MEDLINE (1990 a mayo de 2015), CLINICAL KEYS (2001 a marzo 2015), PUBMED (sin fecha límite) y en las listas de referencias de los artículos. La búsqueda bibliográfica arrojó 43 posibles trabajos deinvestigación, de los cuales se tuvieron en cuenta veinte para desarrollar la presenterevisión. Selección a conveniencia.Resultados: los carbapenémicos como el imipenem, meropenem y ertapenem se convierten en la primera línea para el tratamiento de infecciones debidas a bacterias productoras de betalactamasas de espectro extendido (BLEE). Sin embargo, se ha visto que las quinolonas y los aminoglicosidos pueden mostrar resultados comparables.Desafortunadamente, la resistencia a estos grupos es mayor.Conclusión: con el pasar del tiempo, las bacterias gramnegativas han logrado expresar nuevos y más eficaces mecanismos de resistencia, conllevando a la aparición de IVUs cada vez más resistentes a la terapia convencional. Introducción: el síndrome HELLP históricamente se ha considerado una complicación grave de la preeclampsia, cuyo tratamiento consiste en la estabilización inmediata de la embarazada y el parto. Sin embargo, en pacientes que no responden al tratamiento, la plasmaféresis podría ser considerada una opción terapéutica. El objetivo de este artículo es presentar un caso clínico de una paciente con síndrome HELLP con requerimiento de plasmaféresis y revisar la literatura disponible por considerarse de relevancia clínica.Presentación del caso: paciente de 17 años, primigestante con síndrome HELLP, y requerimiento de plasmaféresis, por ausencia de respuesta al tratamiento. Se revisaron como fuentes de información la historia clínica y la búsqueda de la literatura científica con los términos "HELLP syndrome" “Plasmapheresis” “Treatment“en la base de datos PubMed, sin límite de tiempo; se incluyeron artículos originales, reportes y series de casos.Discusión: el síndrome HELLP se presenta entre el 0.5-0.9% de todas las gestaciones y hasta en un 20% en pacientes con preeclamsia. El tratamiento de primera línea es el parto inmediato, dado que la prolongación del embarazo se ha asociado con un aumento de la mortalidad materna y perinatal; sin embargo en pacientes con síndrome HELLP que no responden al tratamiento habitual dentro de las 24 a 72 horas posteriores al diagnóstico la plasmaféresis es una estrategia terapéutica indicada.Conclusión: la plasmaferesis es una opción de tratamiento que mejora los desenlaces de morbi-mortalidad materna y perinatal, no obstante faltan ensayos clínicos que permitan evaluar el impacto de la plasmaféresis en las gestantes.
