Diante da importância do acompanhamento nutricional na fase infantil, esse estudo tem como objetivo principal avaliar o estado nutricional de crianças de 0 a 5 anos nos últimos 5 anos no estado do Piauí, através da utilização de dados disponíveis no Sistema de Vigilância Alimentar e Nutricional (SISVAN). É um estudo de corte transversal de caráter descritivo que foram avaliadas 2.397.238 crianças menores de cinco anos de idade acompanhadas pelo SISVAN entre os anos de 2017 e 2021, segundo os índices: Peso/Idade, Peso/Altura, Altura/Idade e IMC/Idade. O ano de 2018 e 2019 demonstraram o maior número de atendimento, sexo masculino foi o mais prevalente em todos os períodos e o estado de eutrofia foi prevalente em todo os índices. Entretanto, verificou-se percentuais significativos de prevalência para peso elevado para idade (7,0% a 9,4%), peso baixo para idade (1,0% a 3,4%), magreza (2,3% a 3,5), sobrepeso e obesidade relacionados à idade (2,3% a 3,5%), risco de sobrepeso (15,6% a 17,9%) pelo índice IMC/idade e altura baixa para idade (maior que 12%). Conclui-se que houve uma maior prevalência do estado nutricional de eutrofia para todos os índices antropométricos avaliados, ressaltando a prevalência do estado nutricional relacionado à magreza, sobrepeso e obesidade na população estudada. Com isso, destaca-se a necessidade de políticas públicas mais efetivas para a promoção da saúde e de uma vigilância nutricional mais contínua.
