Envelhecer traz uma série de consequências para o sujeito, dentre elas as perdas cognitivas que podem gerar limitações na autonomia e na independência da pessoa idosa. A falha na memória é uma das queixas mais recorrentes nesta faixa etária, podendo ser consequência de uma série de fatores, dentre eles a depressão e o declínio cognitivo leve (DCL). A avaliação neuropsicológica pode ser utilizada como ferramenta para auxiliar na confirmação de quadros diagnósticos, diferenciando sintomas cognitivos e psiquiátricos. Explorar as dificuldades do diagnóstico diferencial entre a depressão e o declínio cognitivo na pessoa idosa. Estudo de caso único realizado a partir de uma avaliação neuropsicológica com uma idosa de 72 anos com queixa de falhas na memória. Verificou-se que a examinanda não apresentava dificuldades cognitivas proeminentes no momento da avaliação nem dificuldades significativas em suas atividades de vida diária: residia sozinha, fazia as compras da casa, realizava o pagamento das contas sem auxílio e utilizava o transporte público para se deslocar, evidenciando capacidade funcional preservada. No entanto, ela sofreu uma série de perdas em um curto período de tempo: saída dos filhos de casa, diminuição do poder aquisitivo e afastamento de atividades de lazer. A distinção dos sintomas de um DCL e um transtorno depressivo pode ser extremamente complexa. Por vezes, os sintomas se sobrepõem, podendo o DCL ser um fator desencadeante para o transtorno depressivo ou mutuamente. Na ocasião da avaliação, as falhas na memória da examinanda foram melhor explicadas pela depressão. A depressão pode afetar tanto a memória de curto como a de longo prazo, interferindo no processo de aquisição e resgate do que foi memorizado, bem como acarretar falhas atencionais uma vez que, quando o estímulo não é recebido, não pode ser recordado.
