This paper discusses the use of Artificial Intelligence Chatbot in scientific writing. ChatGPT is a type of chatbot, developed by OpenAI, that uses the Generative Pre-trained Transformer (GPT) language model to understand and respond to natural language inputs. AI chatbot and ChatGPT in particular appear to be useful tools in scientific writing, assisting researchers and scientists in organizing material, generating an initial draft and/or in proofreading. There is no publication in the field of critical care medicine prepared using this approach; however, this will be a possibility in the next future. ChatGPT work should not be used as a replacement for human judgment and the output should always be reviewed by experts before being used in any critical decision-making or application. Moreover, several ethical issues arise about using these tools, such as the risk of plagiarism and inaccuracies, as well as a potential imbalance in its accessibility between high- and low-income countries, if the software becomes paying. For this reason, a consensus on how to regulate the use of chatbots in scientific writing will soon be required.
On peut considérer une dizaine d’aspects : nous pouvons choisir entre la centralisation de malades graves dans des institutions de référence et leur maintien grâce à une supervision par télémédecine. Il y a un continuum entre les différents endroits où peut se trouver le malade, du domicile à la réanimation…. La surveillance dans les services hospitaliers est automatisée, avec des appareils de surveillance petits et non contraignants. Le traitement en réanimation aussi largement automatisé. La prise en charge est essentiellement non-invasive, mais le support extracorporel est néanmoins largement utilisé. La douleur est mieux contrôlée, mais les traitements sédatifs ont quasi disparu, et les malades sont mobilisés précocement, et amenés à l’extérieur dès que possible. Leur distraction est assurée et la présence d’animaux domestiques est autorisée. L’automatisation de la surveillance et des traitements permet la réduction des besoins en personnel, qui est aussi plus disponible pour parler aux malades et aux familles, qui sont beaucoup plus présentes. Les décisions éthiques sont facilitées par une meilleure évaluation du devenir du malade et une meilleure compréhension par la société des questions de fin de vie. Les études cliniques évoluent aussi, grâce à une meilleure sélection des malades et l’analyse de large bases de données.
