Testis size and sperm production are directly correlated to the total number of adult Sertoli cells (SCs). Although the establishment of an adequate number of SCs is crucial for future male fertility, the identification and characterization of the factors regulating SC survival, proliferation, and maturation remain incomplete. To investigate whether the IGF system is required for germ cell (GC) and SC development and function, we inactivated the insulin receptor (Insr), the IGF1 receptor (Igf1r), or both receptors specifically in the GC lineage or in SCs. Whereas ablation of insulin/IGF signaling appears dispensable for GCs and spermatogenesis, adult testes of mice lacking both Insr and Igf1r in SCs (SC-Insr;Igf1r) displayed a 75% reduction in testis size and daily sperm production as a result of a reduced proliferation rate of immature SCs during the late fetal and early neonatal testicular period. In addition, in vivo analyses revealed that FSH requires the insulin/IGF signaling pathway to mediate its proliferative effects on immature SCs. Collectively, these results emphasize the essential role played by growth factors of the insulin family in regulating the final number of SCs, testis size, and daily sperm output. They also indicate that the insulin/IGF signaling pathway is required for FSH-mediated SC proliferation.
A decline in human sperm quality and quantity has been reported in numerous Western countries. This observation was also accompanied by an increase in urogenital malformations. The need for epidemiological studies dealing with unbiased populations in order to understand the causes of these observations is obvious. In Switzerland, the large majority of young men are asked to attend a military camp to be drafted into the army. A few weeks before this camp, conscripts were contacted and invited to participate in a large national study on semen quality. The participation was totally voluntary and anonymous. From September 2005 to June 2007, 770 volunteers filled out a questionnaire, underwent a clinical examination and provided sperm, blood and urine samples. Using self-rated health assessments, the observed cohort could be considered as healthy and no testicular cancer was found. Moreover, the testicular volumes, measured using Prader's orchidometry and ultrasonography, were comparable to those already published for young male populations. The median sperm concentration was 47 x 10 6 /ml, which is close to the concentration reported in Denmark, known to have the highest incidence of testicular cancer in Europe. Statistically significant differences were observed between regions with a lower sperm concentration for men residing in the Alps (43 x 10 6 /ml) and in the Zürich area (36 x 10 6 /ml) compared to men from West Plateau (54 x 10 6 /ml) and from the Jura (54 x 10 6 /ml). Such a regional discrepancy could be related to environmental factors, including endocrine disruptors. In order to confirm such regional differences more volunteers from the already studied regions should be studied and other parts of the country should be investigated. The rather low sperm concentration of Swiss young volunteers should be considered as a national health issue and investigated further.
A neonatal piglet model for evaluation of neuroprotective strategies in cardiac surgery in childrenn Zusammenfassung Einleitung Operationstechniken in der neonatalen Herzchirurgie, die mit Kreislaufstillstand in tiefer Hypothermie einhergehen, stellen ein ernstes Risiko hinsichtlich der Beeinträchtigung der psychomotorischen und mentalen Entwicklung dar. Die umfassende Evaluierung des Pathomechanismus solcher Komplikationen ist nur im Tiermodell möglich. Daher untersuchten wir die Veränderungen in der zerebralen Perfusion und Oxygenation sowie die neuropathologischen Veränderungen im Gehirn nach hypothermer extrakorporaler Zirkulation (EKZ) mit verlängertem Kreislaufstillstand in einem neonatalen Tiermodell. Methodik 10 Neugeborenen Ferkeln mit einem Alter unter einer Woche (Gewicht: 2,1 ± 0,5 kg) wird nach dem Anschluss an die EKZ (Fluss 200-250 ml/kgKG/min) und einer homogenen Kühlung auf 14 8C rektaler Temperatur ein Kreislaufstillstand von 60 Minuten induziert. Parameter der Hirnperfusion und Oxygenation werden kontinuierlich durch die Nahinfrarotspektroskopie (NIRS), Blutgasanalysen und den transkraniellen Doppler und punktuell durch die Mikrosphären-technik bestimmt. Nach Reperfusion und Wiedererwärmung werden die Tiere von der EKZ entwöhnt und für weitere 6 Stunden überwacht. Das Gehirn wird dann umfassend histologisch und immunhistochemisch untersucht. Die hypoxischen Neuronen werden quantitativ morphometrisch in der Region des Hippocampus (CA1-CA4 und Gyrus dentatus), des Großhirns, der Stammganglien und des Kleinhirnes erfasst. Es werden Neuronen hinsichtlich der Nekrosen, Apoptosen beurteilt. Die Konzentrationen von Interleukin 8 (IL-8) und Tumor-Nekrosefaktor-Alpha (TNF-a) werden mit Hilfe eines Immunoassay Kitts (Cytoscreen) analysiert. Die biochemische Marker für das Gehirn das Protein S-100 B und die neurospezifische Enolase (NSE) werden mit LIA-Kitts (Byk-Sangtec®, Dietzenbach, Germany) bestimmt. Fünf Arten unterschiedlich fluoreszierender Polysteren-Mikrosphären (FluoSpheres, Molecular Probes Inc., Leiden, Holland) werden zur quantitativen Evaluierung der regionalen Hirnperfusion in den Hauptphasen der EKZ eingesetzt. Ergebnisse Die ersten histologischen Ergebnisse in allen Regionen zeigen ein diffuses perivaskuläres Ödem mit einer Schwellung der astrozytären Endfüße; signifikant hypoxisch veränderte Neurone wurden nach 60-minütigem Kreislaufstillstand nicht gesehen.
