Unter dem Meldebild einer akuten Atemstörung werden RTW und NEF von der zentralen Leitstelle parallel alarmiert. Sie erreichen den Einsatzort nach 7 bzw. 13 Minuten. Der ersteintreffenden RTW-Besatzung bietet sich folgendes Bild: Im Sessel sitzend wird der knapp 80-jährige Patient von dessen Ehefrau und dem herbeigerufenen Hausarzt betreut. Er ist kaltschweißig, blass und wirkt schwer krank. Anamnestisch seien u. a. eine schwere COPD mit Heimsauerstoffpflichtigkeit sowie eine Rechtsherzinsuffizienz bekannt. Vor Eintreffen des Hausarztes und des Rettungsdienstes sei es zu einem Kollaps mit generalisiertem Krampfanfall gekommen, was die Ehefrau veranlasst hatte, Hausarzt und Rettungsdienst zu rufen. Durch das RTW-Team wird unverzüglich die bereits vom Hausarzt eingeleitete Basisversorgung erweitert. Der Patient bleibt zunächst aufrecht sitzend gelagert. Er erhält über eine Nasenbrille 8 Liter Sauerstoff / Minute. Zeitgleich werden EKG und Pulsoxymetrie angeschlossen sowie eine Blutdruckmanschette angelegt. Durch den Rettungsassistenten erfolgt unter schwierigen Bedingungen die Anlage eines venösen Zugangs (G20) am Handrücken. Diese Maßnahme wird durch den anwesenden Hausarzt entsprechend unterstützt. An den venösen Zugang wird eine langsam laufende Vollelektrolytlösung angeschlossen. Im EKG zeigt sich ein AV-Block III. Grades mit einer Kammerfrequenz von etwa 30 Schlägen / Minute (• " Abb. 1). Die zunächst gemessene pSaO 2 beträgt 84 %. In dieser Situation ist der Patient noch bedingt ansprechbar. Das Glasgow-Coma-Scale (GCS) wird mit 11 Punkten dokumentiert. Im Rahmen der Notfalluntersuchung fallen darüber hinaus grobblasige feuchte sowie trockene RGs über der gesamten Lunge auf. Nach dem Eintreffen des Notarztes erfolgt eine kurze Übergabe durch den Hausarzt und das RTW-Team. Vom Notarzt wird zunächst der Versuch einer medikamentösen Steigerung der Herzfrequenz unternommen: Die Gabe von Atropin (insgesamt 1,5 mg) bleibt ebenso wie die 2-malige fraktionierte Gabe von Adrenalin (500 µg) ohne jeden Erfolg auf die Herzfrequenz. Parallel erfolgt die Vorbereitung des transthorakalen Pacers (ZOLL ® M-Serie) sowie die Herstellung der Reanimationsbereitschaft. Zusammenfassung ! Dargestellt wird die präklinische Versorgung bei komplettem AV-Block und Versagen aller medikamentösen und elektrischen Therapieversuche. Im Rahmen der sich anschließenden Reanimation war eine endotracheale Intubation auch durch den erfahrenen Notarzt nicht möglich. Nach anfänglichem Erfolg der Atemwegssicherung mittels Larynxtubus kam es im Verlauf zu einem Versagen auch dieser Methode mit drohender Hypoxie. Vor dem Hintergrund einer Transportzeit von über 30 min wurde eine Koniotomie zur definitiven Atemwegssicherung durchgeführt. Abstract !
ZusammenfassungDer Case Report diskutiert die vorgezogene Intensivtherapie in einem Flughafenterminal. Während einer Reanimation kommt eine mobile Blutgasanalyse zum Einsatz und ermöglich so die zielgerichtete Behandlung „reversibler Ursachen“ vor Ort.
ZusammenfassungWährend einer Reanimation bei Kammerflimmern wurde „Wachheit unter CPR“ beobachtet. Dies führte in der Folge zu einer kritischen Auseinandersetzung und der ethischen Fragestellung, ob eine Sedierung während der Reanimation zum ärztlichen Selbstverständnis gehören müsste, um traumatische Erinnerungen auszuschließen. Im vorliegenden Fall konnte ein junger Reisender am Frankfurter Flughafen erfolgreich wiederbelebt werden und ohne Residuen und Erinnerungen an „seine Reanimation“ nach 11 Tagen die Heimreise antreten.
Diese Kasuistik zeigt, inwieweit eine adäquate notfallmedizinische Therapie Einfluss auf das Outcome, die Verweildauer im Krankenhaus und auf die Verhinderung von Folgeerkrankungen nimmt. Durch den Einsatz von HyperHAES ®1 war eine schnelle Kreislaufstabilisierung zu erreichen. Ohne eine aggressive Volumentherapie wäre eine so weit gehende präklinische Stabilisierung nicht möglich gewesen. An die schnelle, jedoch nur kurzwirksame Stabilisierung wurde eine adäquate konservative Volumenersatztherapie angeschlossen.
