Ein sozialethischer Zugang zeichnet sich dadurch aus, dass er Moral grundsätzlich als ein Phänomen sozialer Beziehungen versteht. Sozialethik, wie sie im Kontext der evangelischen Theologie des 19. und 20. Jahrhunderts entwickelt wurde, versteht sich insbesondere als ethische Reflexion gesellschaftlicher Institutionen und der durch sie realisierten moralischen Güter und der in ihnen Geltung beanspruchenden moralischen Pflichten, die in diesem Beitrag als Ergebnis menschlicher Vergesellschaftungsprozesse gedeutet werden. Entsprechend wird im Folgenden das Pflege-und Gesundheitssystem als diejenigen Institution verstanden, die durch den Schutz der Gesundheit dazu beiträgt, dass Menschen möglichst weitgehend als freie Personen am gesellschaftlichen Leben teilhaben können. Dadurch realisiert es zentrale gesellschaftliche solidarische Schutz-und Wohltunspflichten und trägt sowohl zur Realisierung der Sicherheit als auch der individuellen Freiheit der Personen in der Gesellschaft bei. Anhand von drei Fragestellungen (professionelle Pflege und familiäre Unterstützung, Pflegeeinrichtungen als totale Institutionen, Pflege und ökonomische Interessen) werden exemplarisch die daraus resultierenden Spannungen und ethischen Abwägungsprozesse herausgearbeitet, die auf der grundlegenden Spannung zwischen solidarischen Schutz-und Wohltunspflichten einerseits und der Ermöglichung von individueller, freier Lebensführung durch Institutionen andererseits zurückzuführen sind.
