ABS TRACT Objective: Corticosteroid injections are remarkably effective as a treatment for carpal tunnel syndrome (CTS) in the short term. This study aimed to determine the effects of ultrasound (US)-guided corticosteroid injection with or without hydrodissection on symptom severity, functional status, grip strength, quality of life, and the cross-sectional area (CSA) of the median nerve in CTS. Material and Methods: A prospective cohort of patients with CTS was retrospectively evaluated. A total of 28 patients were randomly selected who received US-guided triamcinolone injection with hydrodissection (3 mL) as the hydrodissection group and US-guided triamcinolone injection (1 mL) as the control group, from the data (case-control ratio 1:1). Outcome measures were the hand grip strength (HGS), CSA of the median nerve, Boston Carpal Tunnel Questionnaire (BCTQ), and Short Form 12. We recorded the assessments at baseline, 1 and 4 weeks after injections. Results: HGS significantly improved, CSA of the median nerve, Symptom Severity Scale, and Functional Status Scale (FSS) scores of BCTQ significantly decreased in both groups throughout the assessment points (p<0.01 for each variable). Percent changes in baseline and 1st-week results between groups showed improvement in the FSS score of BCTQ in the hydrodissection group (-47.46% vs. -13.97%, p=0.016) but this was not available after 4 weeks (-53.25% vs. -33.33%, p=0.053). Conclusion: Corticosteroid injection with hydrodissection did not provide an additional clinical effect except for an improvement in functional scores in 1st week.
Bu araştırmanın amacı, elektromiyografi ile değerlendirilen ve ekstremite travması ile ilişkili travmatik periferik sinir yaralanması (TPSY) olan hastaların etiyolojik ve epidemiyolojik verilerini analiz etmektir. Gereç ve Yöntemler: Bu araştırma, retrospektif ve kesitsel olarak dizayn edildi. Eylül 2016 ile Kasım 2021 arası 3. basamak bir hastanenin ortopedik rehabilitasyon ünitesinde TPSY tanısıyla rehabilitasyon programı uygulanan erişkin hastalar araştırmaya dâhil edildi. Değerlendirme parametreleri yaralanma mekanizması, sinirlerin yaralanma sıklığı, şiddeti ve yaralanma ilişkili kemik kırığıydı. Bulgular: Üç yüz yirmi sekiz hastada toplam 540 TPSY tespit edildi. Hastaların ortalama yaşı 29,20 idi. TPSY'nin en sık nedeni ateşli silah yaralanması (ASY) (%57,9) idi. TPSY'lerin %44,8'i yalnızca üst ekstremitede, %51,5'i yalnızca alt ekstremitede, %3,7'si ise hem üst hem de alt ekstremitede tespit edildi. Peroneal sinir en sık yaralanan sinirdi (%19,1). Üst ekstremitede en sık yaralanan sinirler radial, ulnar ve median sinirler iken; alt ekstremitede en sık yaralanan sinirler peroneal, tibial ve siyatik sinirdi. Yüz yetmiş altı (%53,7) hastada tek sinirde yaralanma saptanırken; 152 (%46,3) hastada çoklu sinir yaralanması mevcuttu. Yaralanan sinirlerin %75'inde parsiyel tutulum, %25'inde ise total tutulum tespit edildi. 226 (%68,9) hastada TPSY ile ilişkili anatomik bölgede kırık saptandı Sonuç: TPSY genç, üretken popülasyonu etkileyebilen bir sorundur. Material and Methods: This study was designed as retrospective and cross-sectional. Adult patients who underwent a rehabilitation program with the diagnosis of TPNI in the Orthopedic Rehabilitation Unit of a tertiary hospital between September 2016 and November 2021 were included. Evaluation parameters were injury mechanism, frequency and severity of nerve injury, and injury-related bone fracture. Results: In 328 patients, 540 TPNIs were detected. The mean age of the patients was 29.20. The most common cause of TPNIs was gun-shot wounds (57.9%). Forty-four-point-eight percent of TPNIs were detected only in the upper extremity, 51.5% in the lower extremity only, and 3.7% in both upper and lower extremities. The peroneal nerve was the most frequently injured nerve (19.1%). The most frequently injured nerves in the upper extremity were radial, ulnar, and median nerves while that in the lower extremity were peroneal, tibial, and sciatic nerves. ÖZET Amaç: YouTube, tıbbi bilgilere erişim için popülerlik kazanan bir platformdur. Ancak, çevrim içi sağlık bilgilerinin kalitesi ve güvenilirliği hakkında endişeler vardır. Bu çalışmanın amacı, YouTube'da De Quervain tenosinoviti ile ilgili yer alan sağlık hizmeti bilgilerinin içeriğini, güvenilirliğini ve kalitesini araştırmaktır. Gereç ve Yöntemler: Kesitsel olarak tasarlanan bu çalışmada, 14 Şubat 2022 tarihinde "De Quervain tenosynovitis" anahtar kelimesi kullanılarak YouTube taraması yapıldı. Videonun yüklendiği tarihten itibaren geçen süre (gün), video süresi (dk), video kaynakları, görüntülenme sayısı ve beğenme sayısı kaydedildi. Videoların değerlendirmesi modifiye DISCERN, Global Kalite Ölçeği (GKÖ) ve Amerikan Tıp Derneği Dergisi (JAMA) benchmark kriterleri ile yapıldı. GKÖ'ye göre 3 kalite grubu (yüksek, orta ve düşük) oluşturuldu. Bulgular: Toplam 60 video tarandı ve dâhil edilme kriterlerini taşıyan 49 video çalışmaya dâhil edildi. YouTube'daki videoların çoğu doktorlar tarafından paylaşılmıştı (n=25, %51). Kalite sınıflandırmasına göre videoların %57'si yüksek kalitede, %23'ü orta kalitede ve %20'si düşük kalitede idi. Yüksek kaliteli videoların %78,6'sı doktorlar tarafından yüklenmişti. Görüntüleme, günlük izlenme, beğeni ve yorum sayısı açısından kalite grupları arasında anlamlı bir fark saptanmadı (tümü için p>0,05). Sonuç: YouTube'da De Quervain tenosinoviti ile ilgili paylaşılan videoların yaklaşık yarısı yüksek kalitededir. However, there are concerns about the quality and reliability of them. The aim of this study was to investigate the content, reliability and quality of healthcare information associated with De Quervain's tenosynovitis on YouTube. Material and Methods: In this cross-sectional study, a YouTube search was performed using the keyword "De Quervain tenosynovitis" on February 14, 2022. The number of days online, video duration (minutes), video sources, number of views, and number of likes were recorded. The evaluation of the videos was performed using the modified DISCERN, Global Quality Scale (GQS) and Journal of the American Medical Association (JAMA) benchmark criteria. Three quality groups (high, intermediate, and low) were created according to the GQS. Results: A total of 60 videos were evaluated and 49 videos meeting the inclusion criteria were included in the study. Most of the videos on YouTube were shared by doctors (n=25, 51%). According to the quality classification, 57% of the videos were high quality, 23% intermediate quality and 20% low quality. Seventy-eight-point six percent of high-quality videos were uploaded by doctors.
