RESUMENLas reacciones cíclicas por progesterona son entidades clínicas muy poco comunes que describen aquellos episodios recurrentes o cíclicos de reacciones alérgicas multisistémicas, que ocurren alrededor de la menstruación y que al parecer, están relacionadas o son exacerbadas durante la fase lútea del ciclo menstrual. Hay poca información al respecto y se encuentran muy pocos reportes de casos que describan dichas reacciones (1,2) . Su diagnóstico es de exclusión y representa un reto considerable desde el punto de vista clínico y del manejo. Los puntos clave para su diagnóstico incluyen la aparición de los síntomas en la fase premenstrual, la desaparición de los mismos con la inhibición de la ovulación y una prueba cutánea positiva a la inyección intradérmica de progesterona. El pilar del tratamiento lo constituye la inhibición de la secreción endógena de progesterona, mediante la supresión de la ovulación con manejo médico, aunque en casos de pacientes con sintomatología severa y refractaria, puede ser necesaria la ooferectomía bilateral. RESUMENAntecedentes: La arteritis de Takayasu es una vasculitis granulomatosa que se presenta principalmente en mujeres jóvenes, con edades de hasta 30 años. Es una condición infrecuente. Se reporta un caso de diagnóstico tardío con imágenes que muestran importante formación de vasos colaterales. Descripción del caso: paciente femenina de 59 años de edad, quien ingresó al Servicio de Medicina Interna del HCG con diagnóstico de síndrome coronario agudo sin elevación del ST (SCASEST). Antecedente de hipertensión arterial (HTA) diagnosticada a los 29 años, sin estudio por causa secundaria. Un ultrasonido doppler de las carótidas demostró ausencia de flujo en ambas carótidas. Una angiotomografía del cuello demostró importante formación de vasos colaterales, que nutrían a la subclavia derecha a través de la región cervical posterior. A la exploración física no se encontró pulsos, excepto por el radial izquierdo y pedio derecho. Conclusión: La arteritis de Takayasu debe ser considerada como parte del diagnóstico diferencial en toda causa secundaria de hipertensión arterial. En este caso particular, el diagnóstico se realizó por los hallazgos tomográficos, que orientan al observar circulación colateral y ausencia de flujo en los principales vasos.
