BackgroundThe role of residents as educators is increasingly recognized, since it impacts residents, interns, medical students and other healthcare professionals. A widespread implementation of resident-as-teacher courses in developed countries' medical schools has occurred, with variable results. There is a dearth of information about this theme in developing countries. The National Autonomous University of Mexico (UNAM) Faculty of Medicine has more than 50% of the residency programs' physician population in Mexico. This report describes a needs assessment survey for a resident as teacher program at our institution.MethodsA cross-sectional descriptive survey was developed based on a review of the available literature and discussion by an expert multidisciplinary committee. The goal was to identify the residents' attitudes, academic needs and preferred educational strategies regarding resident-as-teacher activities throughout the residency. The survey was piloted and modified accordingly. The paper anonymous survey was sent to 7,685 residents, the total population of medical residents in UNAM programs in the country.ResultsThere was a 65.7% return rate (5,186 questionnaires), a broad and representative sample of the student population. The residents felt they had knowledge and were competent in medical education, but the majority felt a need to improve their knowledge and skills in this discipline. Most residents (92.5%) felt that their role as educators of medical students, interns and other residents was important/very important. They estimated that 45.5% of their learning came from other residents. Ninety percent stated that it was necessary to be trained in teaching skills. The themes identified to include in the educational intervention were mostly clinically oriented. The educational strategies in order of preference were interactive lectures with a professor, small groups with a moderator, material available in a website for self-learning, printed material for self-study and homework, and small group web-based learning.ConclusionsThere is a large unmet need to implement educational interventions to improve residents' educational skills in postgraduate educational programs in developing countries. Most perceived needs of residents are practical and clinically oriented, and they prefer traditional educational strategies. Resident as teachers educational interventions need to be designed taking into account local needs and resources.
La elección de una especialidad médica es un proceso que está determinado por múltiples factores, tales como las características demográficas, necesidades y valores del estudiante o su percepción de las distintas especialidades. La elección de la especialidad es importante tanto para los estudiantes como para los sistemas de salud, pues desde los años 90 se ha observado un incremento en la cantidad de subespecialistas y una tendencia a elegir de forma menos frecuente especialidades relacionadas con la atención primaria (medicina general, medicina familiar, medicina interna o pediatría). Otras opciones de formación al terminar la carrera de medicina (investigación, docencia, otro posgrado), dependen fuertemente de la exposición del estudiante a estas opciones durante los años clínicos, al igual que optar por opciones menos conocidas (médicas o quirúrgicas) depende de haber rotado por las mismas durante la formación clínica. En algunos casos el estudiante ya ha tomado la decisión desde el principio (o antes) de la formación médica, como en el caso de aquellos que seleccionan especialidades quirúrgicas, pero en el resto la elección parece tomarse posteriormente. En esta revisión evaluamos los factores relacionados con la elección de una especialidad (con un énfasis particular en México y América Latina), de acuerdo con el modelo de elección de Bland-Meurer.
Introducción: El proceso asistencial y educativo en las instituciones de salud implica diversos actores, uno de los cuales es el médico residente. Este grupo desarrolla simultáneamente dos roles diferentes, como alumnos de programas de especialización de las universidades y como profesionales de la salud empleados por las instituciones hospitalarias. Es necesario reflexionar sobre los avances y obstáculos que tiene el papel docente del médico residente, en el escenario actual de la educación y práctica médica. Objetivo: Proveer una actualización y reflexión crítica sobre el concepto del “médico residente como educador” (MRE). Método: Revisión narrativa de la literatura y ensayo crítico sobre el tema. Discusión: El concepto de MRE ha evolucionado y se ha incorporado en varios programas educativos a nivel internacional, aunque su penetración ha sido menor en nuestro medio. En este documento revisamos el papel del MRE en los escenarios educativos actuales, describimos esfuerzos relevantes para formalizarlo en el currículo de las especialidades médicas, analizamos estrategias para implementar actividades educativas sobre el tema en el entorno de atención de la salud y reflexionamos sobre sus perspectivas para el futuro. Conclusiones: El MRE es un elemento fundamental en el proceso asistencial-educativo de las instituciones de salud y universitarias. Es necesario promover el desarrollo profesional de esta faceta del médico residente, elaborar intervenciones educativas sobre el tema y diseñar instrumentos para evaluar su impacto.
Introducción: La cultura y el clima son ámbitos que configuran las interacciones, los sistemas de creencias, los modos de hacer, los valores y las percepciones de lo que sucede en el contexto organizacional en el que se desempeñan los médicos residentes. Estudiar estas esferas permite adentrarse en la subcultura de los residentes y reconocer lo que facilita o restringe su aprendizaje. Objetivo: Analizar a través de testimonios de residentes sobre incidentes críticos (IC), aspectos de la cultura institucional y el clima de la subcultura que posibilitan o limitan el aprendizaje. Método: Estudio cualitativo e interpretativo cuya muestra consideró los testimonios de incidentes críticos de 135 residentes inscritos en el Plan Único de Especialidades Médicas de la Facultad de Medicina de la Universidad Nacional Autónoma de México (UNAM), en el ciclo Marzo 2012-Febrero 2013, de las cuatro especialidades troncales: Medicina Interna, Cirugía, Pediatría y Gineco-Obstetricia en un hospital privado (36.3%) y uno público (63.7%) de la Ciudad de México. Resultados: Para los residentes del hospital público las jerarquías imperantes en el contexto clínico tuvieron efecto positivo en la eficacia de la atención médica, aunque también hubo abuso de poder, las menciones negativas aludieron a la falta de recursos. En el privado, la seguridad del paciente sobresalió en los relatos. Discusión: La construcción de una visión compartida actuó como soporte de la cultura del grupo, los valores y normas permitieron que la respuesta ante la incertidumbre en un IC fuera más efectiva, fortaleció la cultura institucional y avoreció el aprendizaje. Algunas de las diferencias recursos materiales, la coordinación entre los servicios de especialidades y la autonomía en la toma de decisiones, lo que tuvo distintos efectos en las oportunidades de aprendizaje delos residentes. Conclusiones: La cultura institucional estuvo presente en el clima del subgrupo y se expresó en los IC descritos. Las particularidades en el aprendizaje de ambos hospitales fueron evidentes, sobre todo en la respuesta coordinada de los servicios y en el grado de autonomía en la toma de decisiones.
Assessing teaching performance is important for students' learning. Objective: Assess teaching performance in a medical school, using three strategies. Method: 288 Teachers were studied. The three assessment strategies were: students' academic achievement measured with validated exams, teachers' evaluation by students' opinion (2,656 students) with a validated instrument, and a self-assessment tool. Results: There were significant differences (p≤0.001, ANOVA) among the three assessment strategies. Four different teacher categories were obtained using students' opinion. Conclusion: We identified highperformance teachers, those requiring faculty development and those who do not fulfill appropriately their teaching responsibilities.
