This study aimed at understanding the perception and perspectives of rheumatology trainees about specialist training in India. Rheumatology trainees (Doctorate of Medicine, Diplomate of National Board) in Indian universities (2010 onwards) were contacted to complete a validated e-survey consisting of 41 questions to evaluate the current rheumatology training in India. Of 53 respondents (M:F 3.4:1, mean age 37 years ± 12.7), 81.1% trained at government hospitals, and 15.1% trained at private hospitals. During training period, 37.5% respondents were exposed to 6-7 h of didactics/week. They treated nearly 175 patients (175 ± 35.4) per week and reported a reasonable level of independence in management of patients with common rheumatic diseases (RDs) during their training (7.5 ± 0.7 SD). However, nearly one-third of the trainees were not exposed to basic immunology and laboratory techniques. Similarly, placement in the radiology department was not a part of the curriculum for nearly half of the trainees, 80% were not confident to manage paediatric RDs and soft tissue rheumatism. Almost 60% did not feel comfortable in addressing ancillary care including patient counselling as they had not received formal training. Among the participants, 59% were not satisfied by the current system of assessment, 86.8% suggested for multiple time point-based assessment systems and 45.3% preferred objective and subjective assessment in final examinations. Rheumatology training in India offers notable exposure to patients and independence in managing cases. However, there is an unmet need for improvement in training in the field of laboratory, radiology and ancillary care, and to overhaul assessment system by including objective evaluation.
Lack of adequate nutrition during pregnancy is one of the major risk factors for the development of anemia which further leads to a high-risk pregnancy. Anemia in pregnancy accounts for one-fifth of maternal deaths and is a major factor responsible for low birth weight. The aim of the study was to evaluate the awareness about anemia among the antenatal women attending tertiary care hospitals in South India and also to find out the methods adopted by the women to prevent and correct their anemia. It was a cross-sectional type of study conducted on pregnant women who gave consent. . A majority of the pregnant women were aware of the term anemia and the most common source of their knowledge was their treating physician, followed by ASHA/Anganwadi worker and friends/family. Half of the participants in the study were well aware that anemia was a problem related to blood and a majority knew that it is most common in pregnant women. Around half of the study population knew the correct value of minimum Hb level in a pregnant woman and even a lesser percentage of women knew about the consequence of severe anemia during delivery. Most of the pregnant women were including iron-rich foods normally in their diet. A majority of the respondents noticed a health benefit after taking the iron and folic acid tablets. Thus, there is a need to endorse the IFA tablets and educate the women about the anemia in pregnancy so that the health benefits of the free supplementation program of the government percolates down to even the lower strata of the society.
Strokes are the second leading cause of death and disability worldwide. The brain injury resulting from stroke produces a persistent neuroinflammatory response in the brain, resulting in a spectrum of neurologic dysfunction affecting stroke survivors chronically, also known as post-stroke pain. Excess production of tumor necrosis factor alpha (TNF alpha) in the cerebrospinal fluid (CSF) of stroke survivors has been implicated in post-stroke pain. Therefore, this literature review aims to assess and review the role of perispinal etanercept in the management of post-stroke pain.Several studies have shown statistically significant evidence that etanercept, a TNF alpha inhibitor, can reduce symptoms present in post-stroke syndrome by targeting the excess TNF alpha produced in the CSF. Studies have also shown improvements in not only post-stroke pain but also in traumatic brain injury and dementia. Further research is needed to explore the effects of TNF alpha on stroke prognosis and determine the optimal frequency and duration of etanercept treatment for post-stroke pain.
Lumbar hernia is an uncommon condition that can either be congenital or acquired. Acquired lumbar hernia is further divided into primary, with no identifiable cause, and secondary, occurring due to previous trauma, infection, or surgery. Here, we present the case of inferior lumbar hernia in a 65-year-old Asian male who was a chronic alcoholic and smoker. He presented with a complaint of a longstanding swelling in the right lumbar region for five years and no other associated symptoms. The swelling was reducible, an expansile cough impulse was felt on palpation, and bowel sounds were heard on auscultation. A contrastenhanced computed tomography scan revealed a 6.7 cm defect in the lateral abdominal wall in the right lumbar region with bowel loops, cecum, ascending colon, mesentery, and mesenteric artery seen herniating through the defect. There was a history of an iliopsoas abscess at the same site five years ago, which was treated with incision and drainage. The patient was advised for an open mesh repair but could not be operated upon due to coexisting aortic stenosis and regurgitation. Our impression, from this report, is that a chronic iliopsoas abscess tracking to the inferior lumbar region and the incision and drainage thereof, leading to a weakness in the abdominal wall, may be considered to be a cause of inferior lumbar hernia, with chronic smoking on part of the patient being a significant contributing factor for the abdominal muscle weakness. Therefore prompt and meticulous treatment of an iliopsoas abscess must be done to prevent this complication.

Background: Since the emergence of the SARS COV-19 pandemic, multiple extrapulmonary manifestations of the virus have been reported from around the world. Cardiovascular complications including arrhythmias in patients with COVID-19 have been described in multiple studies. Our aim was to review various case reports detailing the new onset of heart block in COVID-19 patients and to summarise the clinical course of these patients. Methods: We systematically reviewed all reports published and indexed in PubMed, Scopus, and Embase between March 2020 to May 2021, analyzing the relation between the demographics of the patients, pre-existing comorbidities, and the progression of heart block in patients infected with COVID-19. Results: We identified and included in this study 30 relevant articles describing 49 COVID-19 patients with heart block. Among them, 69.3% (n=34) of patients suffered from at least one comorbidity. 36.73% (n=18) of the patients showed spontaneous resolution of the heart block. Conversely, 63.26% (n=31) of the patients had persistent heart block, out of which 16.33% (n=8) and 42.86% (n=21) were implanted with a temporary and permanent pacemaker respectively. The reported mortality rate was 22.45% (n=11) during hospitalization. We noted that 45.45% (n=5) of the patients who died had complete heart block. 24.49% (n=12) of the patients in the studies we reviewed were suspected of having myocarditis. However, none were confirmed with MRI or cardiac biopsy. Conclusions: Additional research is necessary to unearth the mechanism of development of heart block in COVID-19 patients as well as its implications on the clinical course and prognosis. Physicians must be aware of the importance of monitoring patients hospitalized for COVID-19 for arrhythmias including heart blocks, especially in the presence of comorbidities. Early detection can improve the prognosis of the patient.
