The SARS‐CoV‐2 set off a pandemic involving millions of people around the world. The topic of discussion is the possible viral detection in different body fluids than respiratory droplets. Therefore, we evaluated the possible presence of SARS‐CoV‐2 in semen and urine samples. Thirty patients were included in the study aged 35.67 ± 6.84 years. The day after the pharyngeal and/or nose swab of SARS‐CoV‐2 was positive, urine and semen samples were taken from patients, and the presence of SARS‐CoV‐2 was investigated. Laboratory tests and chest CT findings were evaluated simultaneously. SARS‐CoV‐2 was detected in four (13.3%) patients’ semen samples and in seven (23.3%) patients’ urine samples. White blood cell (WBC), neutrophil, C‐reactive protein (CRP), ferritin, alanine transaminase (ALT), lactate dehydrogenase (LDH) and procalcitonin were significantly higher in patients with SARS‐CoV‐2 in semen (p < .05), though no statistical difference was found in urine (p > .05). Patients with severe pneumonia findings in Chest CT images are likely to be PCR positive in semen and urine samples (p = .005, p = .001). SARS‐CoV‐2 was not detected in urine and semen samples of patients after they had recovered (average duration 23 ± 4 days). SARS‐CoV‐2 can be detected in the urogenıtal fluıds of patients with severe clinical conditions and high viral load.
Candida pelliculosa (teleomorph: Wickerhamomyces anomalus) is an opportunistic pathogen that rarely causes fungemia. Here we present two cases with C. pelliculosa fungemia in the pediatric intensive care unit. The fungemia developed as a result of horizontal transmission from a patient who was transferred from another hospital with the same condition. Strains isolated from blood cultures of the two patients were identified as Candida pelliculosa with API ID 32C (bioMerieux France) commercial kit. All strains were resistant to the same antibiotic and random amplified polymorphic DNA analysis type. E-test antifungal MIC levels were found in the order of 0.125 ug/mL, 24 ug/mL, 0.50 ug/mL, and 0.64 ug/mL for caspofungin, fluconazole, voriconazole, and amphotericin-B, respectively. Environmental samples for C. pelliculosa were negative. After measures to control the infection were implemented in the unit, the outbreak ended. In the case of hindering precautions, it should be taken into account that hospital-acquired infections can develop even from rarely encountered non-albicans Candida strains.
Amaç: Boğmaca, Bordetella pertussis'in neden olduğu, tüm yaş gruplarını etkileyebilen, bulaşıcı bir solunum yolu enfeksiyonudur. Aşıya bağlı koruyuculuğun 4-6 yıldan sonra azalması ve ortalama 10 yıl içinde çok düşük düzeye inmesi nedeniyle, gelişmiş ülkelerin aşı şemasına boğmaca aşı pekiştirme dozları eklenmiştir. Boğmaca-yaş spesifik seroprevalansının saptanması, pekiştirme dozlarının gerekli olup olmadığını ve gerekli ise hangi yaşta uygulanmasının doğru olacağını saptamak için en değerli bulgulardan biridir. Bu çalışma bölgemizde genç erişkin bireylerde boğmaca seroprevalansını belirlemek, uygulanan boğmaca aşı takvimine ek rapel dozuna ihtiyacı belirlemek, rapel doz gerekliliği ve hangi yaşta uygulanması konusunda öneriler sunmak amacı ile planlanmıştır. Gereç ve Yöntemler: Ocak 2009-2010 yılları arası, yaşları 13 ile 30 arasında (460 gönüllü) genç erişkin kişilerden toplanan serumda B. pertussis IgG antikor düzeyi ELISA yöntemi ile analiz edildi. Bulgular: Katılımcıların %81,09'unda (n=373) B. pertussis IgG antikorları pozitif, %0,43'ünde (n=2) sınırda değerde IgG antikor düzeyi belirlenirken, %18,91'inde (n=87) IgG antikor düzeyi negatif olarak belirlenmiştir. 13-18 grubunda seropozitivite %81,72 (n=152), 19-30 grubunda seropozitivite %80,66 (n=221) olarak tespit edilmiş olup yaş grupları arasındaki ilişki istatistiksel olarak anlamlı bulunmamıştır (p=0,402). Sonuç: Boğmaca aşısı ile elde edilen koruyuculuğun yaşla giderek azaldığı belirlenmiştir. Bu nedenle mevcut aşı takviminde okul öncesi ve 14-18 yaş arasında asellüler boğmaca aşısı ile pekiştirme dozu uygulanmasının planlanmasının faydalı olacağı inancındayız.
