This article presents modern data on embryonic development and zonal structure of the meta-epiphyseal growth plate in which dysfunction has an important role in the formation of skeletal deformities in children.
Резюме. Реконструкция костей является одной из наиболее актульных проблем ортопедии и травматологии. По данным литературы, поиск оптимальных методов и материалов для костной пластики ведется давно и продолжается в настоящее время. Развитие микрохирургии предоставило новые возможности реконструкции костей, особенно в детской ортопедии. Однако, как и в любом случае, преобладание положительных моментов данного метода сопровождается и наличием недостатков. Поэтому вопрос костной пластики остается актуальным и дискутабельным.Ключевые слова: костная пластика, микрохирургия, аутотрансплантация, пересадка костей у детей.Лечение больных с обширными дефектами длинных трубчатых костей, посттравматически-ми дефектами и деформациями различных тка-ней, сочетающимися с плохой васкуляризацией пострадавшего сегмента, остается сложной про-блемой детской травматологии и ортопедии. К на-стоящему времени нет единого мнения по пово-ду тактики лечения таких больных. Подавляющее большинство хирургов остаются сторонниками поэтапного восстановления целостности по-страдавших структур конечности -замещения дефектов кости, мягких тканей, устранения вто-ричных деформаций. Традиционные методы ле-чения -различные варианты костной ауто-и/ или аллопластики, билокальный остеосинтез по Илизарову и др. часто не дают эффекта или не применимы из-за резкого нарушения трофики поврежденных тканей и дефицита кожных по-кровов в результате тяжелых открытых повреж-дений, в том числе магистральных сосудов и не-рвов, многократных оперативных вмешательств, хронического воспалительного процесса [4][5][6][7][8]. Кроме того, нарушение кровообращения в зоне дефекта или деформации затрудняет процесс ре-генерации костной ткани, приводит к резорбции трансплантата. При использовании традиционных методов (ауто-или аллопластика) увеличивают-ся сроки лечения вследствие длительной пере-стройки трансплантатов, которые к тому же очень чувствительны к инфекции. Лечение компресси-онно-дистракционным методом также требует длительного времени, при этом возрастает риск инфекционных осложнений. Для сокращения сро-ков лечения иногда после дистракции производят замещение ауто-или аллотрансплантатами по ре-генерату [11].Восстановление костной ткани при врожден-ной и приобретенной патологии опорно-двига-тельного аппарата у детей представляет наиболее сложную проблему. Возможности традицион-ных методов лечения данного контингента резко ограничены. Причиной этому являются необхо-димость в многоэтапных операциях, высокая ча-стота больших объемов утраченных тканей, дли-тельная иммобилизация в неудобном положении при стандартных методиках [15]. Основным усло-вием реконструкции опорно-двигательного аппа-рата должно быть использование вмешательств, дающих стабильный функциональный результат и обеспечивающих последующий рост и развитие оперированного сегмента [16].Внедрение микрохирургии в травматологию и ортопедию открыло новые возможности при лечении многих повреждений и заболеваний опорно-двигательного аппарата.
