Background and objectivesType 2 diabetes (T2D) is very prevalent among the elderly. Insulin therapy is often required for glycemic control. The association of starting this therapy with depressive symptoms as well the health-related quality of life (HRQoL) is unknown among the elderly patients.AimsEvaluate the association of starting insulin therapy depressive symptoms as well with HRQoL of elderly people with T2D.Methods36 T2D participants (67.9 % females, age 66.5 years ± 5.1) were recruited, 26 of whom completed the follow-up. Generic (Short-Form 36 Health Survey - SF-36) and specific (Problem Areas in Diabetes - PAID) HRQoL questionnaires, Beck Depression Inventory (BDI), clinical, laboratorial and socio-demographic data were recorded on baseline and 6 months after the beginning of insulin treatment.ResultsThere was a reduction in the BDI score after the use of insulin, which means an improvement in depressive symptoms (Before/After: median – 10.5 / 7; p = 0008). There were no statistically significant differences in HRQoL scores between the two time periods There was also a reduction in HbA1c (Before/After: median – 8.7/7.9). Otherwise, there were no statistically significant differences in: BMI (28.1/28.3); Abdominal circumference:(100.5/99.5) and chronic complications status.ConclusionInsulin therapy in elderly people with type 2 diabetes can lead to an improvement of depressive symptoms and does not seem to affect negatively HRQoL of the participants.
Patients responded satisfactorily to low dose medication, side effects were controlled with simple measures and there has been significant functional capacity improvement. In the conditions of this study, methadone was an effective, safe and low-cost option to treat non-oncologic neuropathic pain.
resUMoO transtorno conversivo é definido pelo Manual Diagnóstico e Estatístico de Transtornos Mentais -Quarta Edição (DSM-IV) por sintomas que afetam a função sensorial ou motora voluntária, assemelhando-se a uma condição neurológica, mas que, após investigação, não podem ser explicados por essa condição, e sim por fatores psicológicos. Em raras ocasiões, o exame neurológico e os exames complementares não são suficientes para estabelecer o diagnóstico do transtorno conversivo. Este artigo descreve um caso para o qual a aplicação de uma técnica de sugestão (torpedeamento) confirmou o diagnóstico de cegueira conversiva e também se mostrou eficaz no tratamento. A sugestão, aliada a psicoterapia e psicofarmacologia, pode ser uma ferramenta valiosa no tratamento desses pacientes. aBstractConversion disorder is defined by Diagnostic and Statistical Manual of Mental DisordersFourth Edition (DSM-IV) as symptoms affecting voluntary motor or sensory function, resembling a neurological condition, and, after investigation, cannot be explained by this condition, but by psychological factors. On rare occasions, neurological examination and laboratory tests are not sufficient to establish the diagnosis of conversion disorder. This article describes a case for which the use of a technique of suggestion (torpedoing) confirmed the diagnosis of conversion blindness and also proved to be effective for its treatment. Suggestion, combined with psychotherapy and psychopharmacology, can be a valuable tool in the treatment of these patients.

Em novembro de 2002 começou a se queixar de "tonteira", vazio na cabeça, fadiga e passou a apresentar isolamento progressivo, irritabilidade, crises de choro diante dos familiares e dificuldades no sono. Nessa época passou a se queixar dos freqüentes episódios de falta d'água em sua residência: "Agora ela está encucada com isso… Não tira da cabeça…", referia a filha. Um mês após o início do quadro, depois de avaliação da clínica médica, iniciou diazepam 10 mg/dia, com melhora parcial da insônia, sem alteração dos demais sintomas.A paciente referiu que no dia 8/2/2003 teve uma discussão com o marido (com quem era casada há 40 anos) sobre o problema da falta d'água.
