There is a small but significant association between HL and eGFR. Providers should use HL-tailored communication strategies in CKD patients. Larger multicentre studies are needed to substantiate this relationship.

Introduction This study assesses validity of self-report for the use of major classes of illicit drugs and opioid-maintenance therapy among pregnant women at a substance abuse treatment program. Methods Analyses used data collected from 83 pregnant women in a prospective cohort study at the University of New Mexico. Study participants with a history of substance abuse were screened and, if eligible, enrolled during an early prenatal care visit. A follow-up interview was conducted shortly after delivery. Self-reported information about drug use later in pregnancy was compared with urine drug screen (UDS) results collected during the third trimester. Simple kappa (k) and prevalence-and-bias-adjusted kappa (PABAK) coefficients were calculated as the measures of agreement. Sensitivity and specificity of self-report for each drug class were estimated using UDS as the ‘gold standard’. Results The sample included a large proportion of ethnic minority (80% Hispanic/Latina and 7% American Indian) and socially disadvantaged (50% < less than high school education and 94% Medicaid-insured) pregnant women. On average, patients had 4.8 ± 3.0 urine drug screens during the third trimester. Sensitivity of self-report was low (<60%) for all classes of illicit drugs; however, marijuana and opioids demonstrated slightly higher sensitivity (57.9% and 58.3%, respectively) than other classes (<47%). Conclusions This study found substantial underreporting for all classes of illicit drugs among pregnant women in a substance abuse treatment program. Rates of underreporting are expected to be higher among the general population of pregnant women.

What is already known on this topic? It is estimated that 13 million people in the United States have latent tuberculosis infection (LTBI). This large number of potential LTBI cases poses a challenge for successful tuberculosis control and elimination. What is added by this report? We examined a novel, collaborative care model using community pharmacies as additional access points for LTBI treatment for patients using combination weekly therapy with isoniazid and rifapentine and directly observed therapy for 12 weeks. What are the implications for public health practice? High completion rates and safe administration of LTBI treatment can be achieved in the community pharmacy setting.

The New Mexico Rural Health Interdisciplinary Program was initiated in 1990 to meet the need for interdisciplinary health professional student training and to advance the recruitment and retention of health professionals in rural healthcare shortage areas. This paper describes the New Mexico programme, including its interdisciplinary problem‐based learning format and mechanisms for generating community involvement, and reports the evaluation findings and lessons learned from 10 years of programme experience. The New Mexico programme utilizes student‐centred, problem‐based learning (PBL) as an educational format and currently involves 12 healthcare disciplines in six communities throughout the state. Evaluation has focused on differences in student attitudes about and confidence in interdisciplinary and rural practice prior to and upon completion of the programme, as well as change in student expectations of consulting with other health professionals. A pre‐test/post‐test survey was used to assess change. The growth of the programme provides some evidence of its success. Results indicate a consistent change in attitude of participants towards greater confidence to work in rural settings and with interdisciplinary teams and an increased expectation to consult with other healthcare professionals. The lessons of this 10 year training experience include the compatibility of PBL to an interdisciplinary training model, the importance of scheduling to facilitate optimal participation, the need for a dedicated core faculty, the worth of informal as well as formal interdisciplinary student interaction, the need to cultivate community relationships and support and the challenge of programme evaluation.

This article examines the relationship among diabetes-related stress, appraisal, coping and depressive symptoms in patients with type 2 diabetes mellitus (T2DM) using the transactional model of stress and coping (TMSC) as the theoretical framework. In this cross-sectional study, a convenience sample of 201 patients with T2DM was recruited from three outpatient clinics. Patients with depressive symptoms reported significantly more diabetes-related stress than patients without depressive symptoms. The results of path analysis suggest that patients who experience greater diabetes-related stress or greater depressive symptoms have a negative appraisal of their diabetes. Negative appraisal is, in turn, associated with greater use of avoidance, passive resignation and diabetes integration coping and lesser use of problem-focused coping. Avoidance, passive resignation and diabetes integration coping are, in turn, related to greater depressive symptoms or greater diabetes-related stress. Overall, the results of this study support the TMSC as a framework to elucidate the relationships among diabetes-related stress, appraisal, coping and depressive symptoms in patients with T2DM. However, given the cross-sectional nature of the study, we are unable to elucidate the directionality of the relationship between stress and depressive symptoms. Implications of the findings and the need for longitudinal studies to evaluate these relationships are discussed.
