BACKGROUND: Laboratory studies suggest applying positive pressure without endotracheal suction during cuff deflation and extubation. Although some studies reported better physiological outcomes (e.g. arterial blood gases) with this technique, the safety of positive pressure extubation technique has not been well studied. The aim of this study was to determine the safety of the positive-pressure extubation technique compared with the traditional extubation technique in terms of incidence of complications. METHODS: Adult subjects who were critically ill and on invasive mechanical ventilation who met extubation criteria were included. The subjects were randomly assigned to positive-pressure extubation (n ؍ 120) or to traditional extubation (n ؍ 120). Sequential tests for noninferiority and, when appropriate, for superiority were performed. Positive pressure was considered noninferior if the upper limit of the CI for the absolute risk difference did not exceed a threshold of 15% in favor of the traditional group, both in per protocol and intentionto-treat analyses. A P value of <.05 was considered significant. RESULTS: A total of 236 subjects were included in the primary analysis (per protocol) (119 in the positive-pressure group and 117 in the traditional group). The incidence of overall major and minor complications, pneumonia, extubation failure, and reintubation was lower in the positive-pressure group than in the traditional group, with statistical significance for noninferiority both in the per protocol (P < .001) and intention-to-treat (P < .001) analyses. The lower incidence of major complications found in the positive-pressure group reached statistical significance for the superiority comparison, both in per protocol (P ؍ .03) and intention-to-treat (P ؍ .049) analyses. No statistically significant differences were found in the superiority comparison for overall complications, minor complications, pneumonia, extubation failure, and reintubation. CONCLUSIONS: Positive pressure was safe and noninferior to traditional extubation methods. Furthermore, positive pressure has shown to be superior in terms of a lower incidence of major complications. (ClinicalTrials.gov registration NCT03174509.
Objetivo: Conocer la prevalencia de los motivos de consulta y las características de los sujetos que ingresaron a un consultorio externo de kinesiología de un hospital público de la Ciudad Autónoma de Buenos Aires (CABA). Materiales y método: Estudio observacional, descriptivo, retrospectivo y transversal. Se recolectaron los datos de los sujetos ingresados al consultorio externo de la unidad de kinesiología atendidos durante el turno tarde. Para evaluar la distribución de las variables, se utilizó el test Kolmogorov-Smirnov. Resultados: Ingresaron un total de 1029 sujetos. El diagnóstico kinésico más prevalente fueron las algias inespecíficas y degenerativas con un total de 514 sujetos (49,9%), con una relación 2:1 a favor de las mujeres. El diagnóstico médico más prevalente fue el dolor lumbar. Del total de sujetos, 528 recibieron el alta kinésica, el 60% fue otorgada entre la sexta y décima sesión. Los sujetos desempleados tuvieron la tasa más alta de abandono, la cual fue del 60%. El grupo operarios fue la ocupación más prevalente. Conclusión: Este estudio permitió conocer la prevalencia de los motivos de consulta y las características de los sujetos que ingresaron a la unidad de kinesiología de un hospital público de CABA. El motivo de consulta más frecuente fue la lumbalgia. La mayor parte de la muestra estuvo conformada por sujetos de sexo femenino, de nacionalidad argentina, entre 41 y 64 años. En ocupaciones, la categoría operarios fue la más prevalente.
Introducción: La oxigenoterapia a través de cánula nasal de alto flujo (CAFO) disminuye las tasas de intubación y dificultad respiratoria, aumenta la oxigenación y el confort del paciente. Su utilización en el tratamiento de IRAB es bien documentado mientras que en neumonía es discutida. La investigación sobre su uso fuera de las unidades críticas es escasa. Objetivo: Describir las características y evolución de los niños que requirieron oxigenoterapia mediante CAFO para el tratamiento de IRAB en la sala de internación general pediátrica de un hospital de agudos, haciendo un sub analisis en aquellos con Neumonia. Materiales y métodos: Estudio observacional, descriptivo y retrospectivo. Se incluyeron sujetos con diagnóstico de IRAB, que recibieron terapia CAFO, en la sala de internación general pediátrica, entre el 1/06/18 y 30/11/18. Se registraron variables demográficas, variables clínicas y la estadía hospitalaria. Resultados: Veintinueve sujetos fueron analizados. La mediana de edad fue de 5 meses (RIQ= 1,5 - 11). El motivo de ingreso más frecuente fue bronquiolitis (bql). La media de días de utilización de CAFO fue de 4,5 días (DE=1,8), la media de días de internación fue de 6,3 (DE= 2,6). Dos sujetos requirieron derivación a unidades de cuidados intensivos. Conclusiones: Este estudio describe las características demográficas, evolución clínica, días de internación y uso de CAFO, destino al alta de niños tratados en una sala de internación general. En el subgrupo de sujetos con neumonía, se encontró un comportamiento similar al de los sujetos con bronquiolitis.
