Variatie tussen patiënten in de mate van remming van hersenactiviteit verklaart mogelijk waarom medicijnen bij de één wel en bij de ander niet werken. Analyse van het EEG tussen aanvallen in biedt inzicht in de mate van verstoorde remming. Hier beschrijven we recent gestart translationeel preklinisch en klinisch onderzoek naar een EEG-biomarker als mogelijke indicator voor effectiviteit van medicatie.
Hoog Frequente Oscillaties (80-500 Hz) worden al bijna twintig jaar genoemd als een nieuwe en betere EEG-biomarkervoor het afgrenzen van het epileptische hersenweefsel dan de nu gebruikte pieken. Hier wordt het resultaat gepresen-teerd van een unieke en zeldzame randomized controlled trial met als doel het verzamelen van prospectieve bewijslastvoor het klinische gebruik van hoog frequencte osccillaties als biomarker tijdens epilepsiechirurgie.
Het elektrocorticogram wordt toegepast als hulpmiddel bij de afgrenzing tussen epileptogeen weefsel en gezondweefsel tijdens epilepsiechirurgie. Mensen met een focale corticale dysplasie die intra-operatieve elektrocorticografieondergaan hebben vaak een kenmerkend elektrocorticogram. In deze studie worden de lokaliserende en tijd-gerela-teerde eigenschappen van met het elektrocorticogram gemeten pieken en hoogfrequente oscillaties geanalyseerd.
