Objective. To describe the transformation of decisive moments that arise within the nurse-patient and family caregiver interaction to turn them into moments of care capable of favoring adaptation. Methods. In a high complexity hospital in the city of Bogotá (Colombia), a "nursing methodological research"-type study was conducted. It was developed in five stages: 1) identification of the institutional route of patients and their caregivers and, within it, the moments of encounter with nursing; 2) typical day of the nurse; 3) analysis of the nurse-patient and family caregiver encounters; 4) literature review on how to strengthen the nurse-patient and family caregiver relationship; and 5) proposal to transform decisive moments into moments of care. RESUMENCon el propósito de reflexionar acerca del valor del cambio de turno como momento y eje central del cuidado de enfermería, se realizó una revisión de la literatura en idioma español, inglés y portugués, a partir de los descriptores: cambio de turno, informe de turno, pase de guardia, traspas de información, y comunicación (circunscrita a informes de enfermería). La búsqueda se hizo en las bases de datos SciELO, Ovid, Science Direct, Redalyc, Publindex, Lilacs, EBSCOhost Academic Search Premier, pubmed y Dialnet; se revisaron un total de 80 artículos de los cuales se seleccionaron 50 que contenían los criterios de inclusión.Los resultados de la búsqueda evidenciaron gran cantidad de información frente al concepto, tipo, características y barreras, destacando la importancia que tiene para la disciplina comprender su significado en la práctica diaria.En conclusión, se debe reflexionar sobre la importancia del permanente crecimiento profesional; los cambios los procesos y procedimientos pero aún más ratificar que el informe no es una oportunidad de un momento, es necesario educar, realizar seguimiento, evaluación y retroalimentación para permitir mejoras de los grupos y fomentar su práctica diaria. La revisión de literatura muestra una tendencia en estudios cualitativos y en menor proporción cuantitativos, además de escasa investigación en mejores prácticas, evaluación de las mismas y la eficacia de los protocolos o guías estructuradas, que contribuyan a través de la investigación a la práctica diaria al cuidado con calidad y calidez. Background: Technology reduces the nursing workload, improve the quality care processes, patient's safety, and avoid staff burnout. Innovative technologies are disrupting healthcare systems by improving the efficiency of processes and management. There is a discussion on the benefits, challenges, and barriers of these technologies and considering human factors of nursing management. Methods: To analyse the nursing workload models, the predictors of nursing burnout and outcomes, the new technologies and its acceptance for nursing care management based on the literature. An integrative literature review is performed. Scopus, Scielo, PUBMED, and CINALH databases were searched to perform an integrative review following PRISMA guidelines. Articles published from January 2016 to December 2020 were included. Quality appraisal was performed using the Crowe Critical Appraisal Tool version 1.4 (CCAT). Two reviewers independently examined the title and abstract for eligibility according to the inclusion and exclusion criteria. Quality appraisal was performed using the Crowe Critical Appraisal Tool version 1.4 (CCAT). Results: Initially 2,818 articles were potentially relevant. After following the PRISMA Guidelines, 35 studies were included in the review. Four themes appeared: Nursing workload models; Predictors of nursing burnout and outcomes; Information technologies and technological means for management; Technology acceptance. Conclusions: Technology has the potential to improve care management by estimating nurse workload in ICUs and non-critical units, but scientific evidence is more detailed in the former type of services. The literature provides insights about the factors that factors and the barriers that promote the technology acceptance and usability. We did not find studies comparing technologies and no scientific evidence proving improvements in care.
Introducción. Conocer y atender debidamente las necesidades de un ser humano en medio de su vulnerabilidad continúa siendo un reto vigente. Objetivo. Describir la transformación de la “Atención a las necesidades básicas” en un momento de cuidado de enfermería para favorecer la adaptación del paciente y su cuidador familiar durante la hospitalización, en la Clínica Universidad de La Sabana, para el periodo 2018-2019. Método. Esta es una investigación metodológica en enfermería, realizada bajo lineamientos del Modelo de Enfermería Universidad de La Sabana, que incluyó cuatro fases. Resultados. Es preciso que la atención a las necesidades básicas sea respaldada en la mejor evidencia disponible, y con un trato humanizado. Hacer visible este proceso permitió medirlo y mejorarlo. Conclusiones. Para brindar un cuidado humano y seguro, no basta la buena intención, es necesario garantizar dos condiciones; por un lado, el compromiso requerido para soportarlo con la mejor evidencia disponible, aumentando la calidad del cuidado y la calidez o tono humano; y, por otro, el respeto y reconocimiento de la dignidad de las personas, favoreciendo el vínculo con ellas y su autonomía, lo que a su vez repercute positivamente en el desarrollo de la enfermería.
