Introdução: Estima-se que 10 milhões de gestações indesejadas em adolescentes ocorrem todos os anos em países em desenvolvimento, tendo consequências sociais, econômicas e na saúde. Ações educativas, que utilizam conhecimentos estruturados sem necessidade de alta tecnologia, representam tecnologias leve-duras, sendo sua avaliação importante no fornecimento de subsídios para a tomada de decisão em saúde. Desta forma, há a necessidade de se identificar as estratégias focadas na postergação do início da atividade sexual por adolescentes, que podem contribuir na redução dos casos de gestação precoce. Objetivos: Sintetizar as evidências científicas disponíveis sobre estratégias focadas na postergação do início da atividade sexual de adolescentes na prevenção da gravidez. Métodos: Foram realizadas buscas sistemáticas nas bases Medline, Scopus, PsycINFO, CINAHL Plus, ERIC e LILACS, a fim de identificar estudos intervencionais publicados a partir de 2010 em caracteres romanos, avaliando intervenções focadas no retardo do início da atividade sexual de adolescentes na redução de casos de gestação. Dois autores independentes realizaram a seleção dos estudos e extração de dados. Resultados: Foram recuperados 7161 artigos nas bases de dados, após exclusão das duplicatas, dos quais 128 foram incluídos para leitura na íntegra. Destes, 51 foram selecionados para integrarem as análises. A maior parte dos estudos foi conduzida nos Estados Unidos (73%), seguido de países africanos (14%). Do total de estudos, 76% foram experimentais e 24% quase-experimentais, com tamanho amostral variando de 24 a 19289 participantes. A grande maioria das intervenções (74%) foi abrangente, incluindo orientações sobre saúde sexual e reprodutiva, enquanto 26% delas focaram exclusivamente na postergação do início da atividade sexual. Os componentes das estratégias incluíram aulas expositivas, debates, dramatizações, jogos eletrônicos, oficinas, diários e simuladores infantis realistas. Destaca-se que a descrição das intervenções muitas vezes não estava satisfatória. As intervenções foram providas predominantemente em escolas (86%), de forma presencial (90%), e em grupos (73%), por professores (25%), facilitadores (22%), educadores em saúde (20%) e profissionais de saúde (12%). O beneficiário da intervenção foi exclusivamente o adolescente em 90% dos estudos. Em 94% dos estudos foi avaliado o desfecho de abstinência sexual ou intenção em manter-se abstinente, enquanto 20% deles avaliaram redução no número de casos de gestação. Conclusão: Os estudos incluídos demonstraram estratégias educativas heterogêneas, incluindo diferentes componentes, conduzidos em distintos locais e providos a uma diversidade de adolescentes por distintos protagonistas. Sendo assim, destaca-se a importância dos autores descreverem de forma detalhada as intervenções realizadas, bem como torna-se relevante identificar quais componentes das intervenções são mais efetivos na prevenção da gravidez na adolescência.
