The purpose of this study was to analyse vaccine refrigerator and vaccine management in primary care and to identify physician- and practice-related influencing factors. Adequate cooling of vaccines in a temperature range of 2-8°C is essential to assure vaccine effectiveness. Studies from various countries have demonstrated cooling chain problems. We surveyed general practitioners about the quality of their vaccine refrigerator and vaccine management and aimed at identifying physician- and practice-related influencing factors. A cross-sectional, web-based questionnaire survey was performed among 3 physician populations in primary care: a 10% random sample of general practitioners (n=954), all teaching physicians of the Universities Duisburg-Essen (n=221) and Halle-Wittenberg (n=92). Surveyed were items on the following 6 aspects: (1) responsibilities within practice teams, (2) vaccine ordering and storing, (3) criteria for the vaccine pre-selection, (4) stocking system inside the refrigerator, (5) wrapping, and use of stocking boxes, (6) refrigerator and temperature control. The quality indicator "comprehensive refrigerator management" was defined to include 4 aspects: (1) separate refrigerator, (2) written temperature documentation (temperature-logbook), (3) regular storage control (wrapping, temperature and expiration date), and (4) storage in original wrappings. A total of 278 physicians participated in the survey (22%). Of these, 80% had a separate refrigerator, 52% reported written temperature documentation, 93% documented regular storage control addressing vaccine wrappings, temperature and expiration dates, and 95% reported vaccine storage in original card box wrappings. A "comprehensive refrigerator management" was realised by 42% of the practices. This indicator was reached more frequently by practices with 3 or more physicians (p=0.01) and those with an additional qualification in travel medicine (p=0.036). Our survey showed good results for most aspects of the refrigerator and vaccine management but room for improvement with regard to temperature documentation.
Jak wynika z danych epidemiologicznych, w ostatnim czasie obserwuje się wzrost częstości występowania raka gruczołowego szyjki macicy, szczególnie u młodych kobiet. Skuteczność rutynowo stosowanych metod przesiewowych w wykrywaniu zmian prekursorowych jest w przypadku adenocarcinoma zdecydowanie niższa niż w przypadku zmian płaskonabłonkowych. Wielu autorów zwraca uwagę na mniej korzystne przeżycia chorych na raka gruczołowego w porównaniu z chorymi na raka płaskonabłonkowego. Rak trzonu macicy jest jednym z najczęstszych nowotworów złośliwych u kobiet w Polsce; dotyczy głównie pacjentek w wieku około-i pomenopauzalnym. Wczesne objawy tego nowotworu to nieprawidłowy schemat krwawień z dróg rodnych u kobiet w wieku reprodukcyjnym lub krwawienie po menopauzie. Skuteczność walki z rakiem endometrium zależy od wyboru rodzaju i sposobu leczenia. W pracy przedstawiono opis przypadku 31-letniej pacjentki, u której po konizacji szyjki macicy rozpoznano raka gruczołowego szyjki macicy. Ze względu na powyższy wynik histopatologiczny pacjentkę zakwalifikowano do operacji sposobem Wertheima. Zabieg i przebieg pooperacyjny bez powikłań. Ostateczny wynik histopatologiczny wskazywał na raka gruczołowego jasnokomórkowego trzonu macicy (adenocarcinoma clarocellulare). Na podstawie zaprezentowanego opisu przypadku omówiono trudności diagnostyczne dotyczące różnicowania raków gruczołowych szyjki i trzonu macicy oraz prawidłowe sposoby postępowania w obu typach nowotworów. Należy mieć na uwadze, że ostateczny wynik histopatologiczny wpływa na postępowanie pooperacyjne. The efficacy of routinely used screening methods in detecting glandular precancerous lesions is significantly lower than in planoepithelial lesions. Many authors emphasize a worse outcome in cervical adenocarcinoma patients when compared with the patients with squamous cell cancer. Endometrial cancer is one of the most common gynecologic malignancies in Poland-affecting menopausal women. An early symptom of this cancer is an abnormal bleeding pattern in women of childbearing age or bleeding after menopause. The selection of the type and methods of endometrial cancer treatment are crucial factors influencing the final effectiveness of treatment. The article presents a case report-patient (age 31) who after cervical conization was diagnosed with cervical adenocarcinoma. Based on the diagnosis, the patient was qualified to Wertheim's operation (radical hysterectomy with pelvic lymphadenectomy with paracervical tissue). There were no complications during or after surgery. The final histopathological report proved endometrial clearcell adenocarcinoma. Rak jajnika jest coraz częściej rozpoznawany u kobiet w wieku rozrodczym. Powstaje wtedy pytanie, co dalej. Jeżeli choroba zostanie wykryta we wczesnym stadium zaawansowania, to pięcioletnie przeżycia wynoszą średnio 75-80%. W niniejszej pracy zaprezentowano przypadek 21-letniej pacjentki z rozpoznanym pooperacyjnie wczesnym rakiem jajnika, która zdecydowała się na operację oszczędzającą i urodziła dziecko. Po kilku latach doszło do wznowy nowotworu. Kobieta w wieku rozrodczym ze zdiagnozowanym rakiem jajnika powinna mieć wykonane oznaczenie mutacji BRCA i odbyć konsultację onkologiczną. Ponieważ po operacjach oszczędzających przeprowadzanych z powodu raka jajnika w niskim stopniu zaawansowania w 9-15% przypadków występuje nawrót choroby, niezwłocznie po zrealizowaniu przez pacjentkę planów prokreacyjnych powinno się wykonać usunięcie macicy i przydatków. In the case of an early-stage cancer, 5-year survival rates range from 75 to 80%. We are presenting here the case of a 21-year-old patient with postoperatively diagnosed early-stage ovarian cancer, who decided to undergo conservative surgical treatment, and was later able to conceive and bear a child. Several years on, she suffered a relapse. Patients of childbearing age with diagnosed ovarian cancer should be tested for BRCA mutation, and receive oncology consultation. As 9-15% of patients suffer relapse after conservative surgery conducted at an early stage of the disease, full resection is essential immediately after the patient's childbearing plans have been completed.
