Introduction : En éducation thérapeutique, les professionnels sont soucieux d'associer les patients à la conception des programmes éducatifs afin de se rapprocher de leurs préoccupations et de mieux préciser les apprentissages nécessaires. Objectifs : Notre étude explore la manière de définir des objectifs d'éducation thérapeutique, selon une analyse de besoins éducatifs en associant le patient et son entourage, à partir d'un outil de repérage conçu à cet effet. Méthodes : L'étude s'est déroulée dans un centre d'éducation du patient, auprès de 12 patients à risque cardiovasculaire accompagnés de leur conjoint, au cours d'entretiens diététiques. Le recueil d'opinion de l'utilité et de la faisabilité de l'outil a été fait auprès des patients et conjoints par questionnaire, et auprès de trois professionnels du Pôle par un entretien collectif semi-dirigé. Résultats : Les résultats montrent un degré de concordance variable entre les objectifs déterminés par le patient, son conjoint et le soignant. Toutefois, l'étape de négociation des objectifs qui suit leur identification a permis certaines redéfinitions à l'origine d'un programme éducatif personnalisé, prenant en compte les priorités du patient, les besoins du conjoint et le point de vue du soignant. Conclusion : L'analyse de la perception de son utilité nous conduit à proposer des améliorations sur sa forme pédagogique et à mieux définir les conditions précises de son utilisation dans le parcours éducatif du patient. Surtout, cet outil met en évidence la relation en jeu entre le patient et son conjoint dans la gestion de la maladie et du traitement. Introduction: In therapeutic education professionals care about involving patients in designing their educational programs in order to be as close as possible to their concerns and to clarify better the necessary learning. Objectives: This study explores the way to specify the objectives of therapeutic education by the analysis of educational needs involving the patient and his relatives with a referencing tool conceived to this purpose. Methods: The study took place in a patient education center next to 12 patients with cardiovascular risks and their spouses during diet sessions. To evaluate the perception of utility and feasibility of the tool, patients and spouses answered questionnaires and the three center professionals had a semi-structured discussion. Results: The results show a varying concordance between the objectives determined by the patient, those of the spouses and the ones of the health professionals.
