This preliminary study suggests that robot-guided drilling of a minimally invasive approach to the cochlea might be feasible, but further improvements are necessary before any clinical application becomes possible. Where the width of the facial recess is less than 2.5 mm, the chorda tympani nerve and the ossicles are at risk.
The aim of this study was to evaluate artifacts produced by cochlear implants (CI) during 3.0 Tesla (T) magnetic resonance imaging of the brain using different sequences on phantom and cadaveric specimens. A phantom and three cadaveric specimens with CIs were imaged using a 3.0 T clinical scanner. Artifacts were analyzed quantitatively and according to the sequence used. Different brain regions were evaluated for image distortion and limitation of diagnostic significance. In cadaver studies, all sequences generated signal-void areas around the implant. In T2-weighted sequences, additional periodic shadowing was discovered. Anatomical structures of the brain on the contralateral side of the CI were for the most part undistorted. At 3T, artifacts around CIs with non-removable magnets compromise image quality of the nearby brain regions and diagnosis of brain lesions is limited. In the contralateral hemisphere, diagnostic accuracy is only marginally limited.
Contact endoscopy offers valuable support for the evaluation of oropharyngeal, hypopharyngeal, and laryngeal mucosa. Contact endoscopy can be a useful contribution to rapid intraoperative evaluation of mucosal alterations for early diagnosis of tumors and might reduce diagnostic biopsy sampling. Even so, it does not replace biopsy sampling.
The aim of this study was to examine the functional hearing results regarding speech perception and auditory sound localization in a high-resolution directional hearing setup following implantation with a new bone conduction device (MED-EL Bonebridge, Innsbruck, Austria). In addition, we assessed the patient acceptance of the Bonebridge system using a questionnaire. The study design is retrospective study. The setting is University Hospital Frankfurt. 18 patients implanted with a Bonebridge device from May 2012 to January 2015 were participated in this study. Speech perception in quiet was tested with the Freiburg monosyllable test at a presentation level of 65 dB SPL. Speech perception in noise was tested post-operatively with the Oldenburg sentence test (OLSA) in best-aided condition. We assessed auditory sound localization with a high-resolution directional hearing setup. To evaluate the acceptance by patients using the Bonebridge in daily life, we used a modified questionnaire. The overall average of functional hearing gain (n = 18) was 29.3 dB (±20.7 dB). Speech perception of monosyllabic words in quiet improved by 20.7% on average, compared with the pre-operative aided condition. Mean speech reception thresholds (SRTs) of the Oldenburg sentence test (OLSA) improved significantly from -3.8 dB SNR (range -5.7 to 5.8 dB SNR) to -5.2 dB SNR (range -6.3 to -0.6 dB SNR) after implantation. Regarding localization abilities, no significant difference was found between the unaided and aided conditions following Bonebridge implantation. A survey of patients' acceptance and handling of the Bonebridge implant in daily life revealed high patient satisfaction. All patients accepted and benefited from the implanted system. No infections or adverse surgical effects occurred. Speech perception significantly improved in quiet and in noise. No significant difference in sound localization was observed. Acceptance of the Bonebridge implant, tested with a modified questionnaire, was high.
Patients carrying CIs with non-removable magnets should not enter a 3.0T MRI device in a routine clinical setup. Under special conditions (angle between the two magnets less than 80 degrees) imaging in a 3.0T MRI may be possible without harming the patient or the implant.
The study confirms the functionality and reliability of the automated insertion tool for insertion of preformed CI. Improved insertion strategies considering patient-specific anatomy become possible.
Zusammenfassung
Einleitung Seit März 2020 wurden infolge des exponentiellen Anstiegs der Infektionen mit SARS-CoV-2 in Deutschland/Europa die Krankenhäuser durch das Gesundheitsministerium aufgefordert, verstärkt Kapazitäten zur intensivmedizinischen Behandlung zu schaffen. An vielen Kliniken wurden deshalb kurzfristig massive Umstrukturierungen und Ressourcenverschiebungen vorgenommen. Ziel der vorliegenden Arbeit war die Erfassung der Auswirkungen der SARS-CoV-2-Pandemie auf die universitäre Hals-Nasen-Ohren(HNO)-Heilkunde in Bezug auf die Krankenversorgung.
Material und Methoden Eine Online-Befragung wurde an alle Direktorinnen und Direktoren der 39 Universitäts-HNO-Kliniken in Deutschland versendet. Die Fragen bezogen sich auf den definierten Zeitraum vom 15.03. bis 15.04.2020 und die Beantwortung wurde mithilfe des Online-Befragungstools „SurveyMonkey“ durchgeführt. 87 Fragen bezogen sich auf allgemeine Angaben, die Krankenversorgung und die strukturellen Auswirkungen in der jeweiligen Einrichtung.
Ergebnisse An der Befragung beteiligten sich alle Direktorinnen und Direktoren der 39 Universitäts-HNO-Kliniken in Deutschland. Die erhobenen Daten belegen die erheblichen Auswirkungen auf organisatorische, strukturelle und medizinische Aspekte der Krankenversorgung. So gaben die befragten Kliniken einen Rückgang der ambulant behandelten Fälle um 73,8 % auf 26,2 ± 14,2 % und der operativen Behandlungen um 65,9 % auf 34,1 ± 13,9 % an. Im Gegensatz hierzu erfolgte in 80 % der Kliniken eine unveränderte oder sogar vermehrte Notfallbehandlung und in mehr als 90 % eine unveränderte oder sogar vermehrte operative Behandlung von Notfällen. In allen Kliniken war die ambulante und operative Notfallbehandlung während der Pandemie durchgehend gewährleistet. Insgesamt wurden an allen HNO-Universitätsklinken circa 35 000 ambulante und circa 12 000 operative Fälle verschoben. Infolge der akuten strukturellen Veränderungen wurde in einzelnen Bereichen der Krankenversorgung die potentielle Gefahr einer Unterschreitung geltender Behandlungsstandards gesehen.
Diskussion Die Bewertung der Auswirkungen der SARS-CoV-2-Pandemie fällt heterogen aus. Kritisch wird mehrheitlich die Gefahr der Unterschreitung geltender Behandlungsstandards oder Leitlinien bewertet. In der Phase der exponentiellen Zunahme der Infektionszahlen mussten teilweise erhebliche Veränderungen von Behandlungsabläufen aus nachvollziehbaren Gründen akzeptiert werden. Mit den aktuell deutlich reduzierten Infektionszahlen darf aber eine Unterschreitung von Behandlungsstandards und Leitlinien nicht verstetigt und toleriert werden.
Zusammenfassung Die hier dargestellte Untersuchung zeigt ein differenziertes Bild hinsichtlich der Auswirkungen der SARS-CoV-2-Pandemie auf die ambulante, stationäre und operative Krankenversorgung an den Kliniken der universitären HNO-Heilkunde in Deutschland und verdeutlicht die Bedeutung der Universitätskliniken für die Krankenversorgung während dieser kritischen Phase.
