Double toenail is a rare abnormality that usually affects the fifth toe. It is considered as an inherited condition that may represent an incomplete form of polydactyly. Underlying bone abnormalities should be ruled out. We present a case of a double toenail of the second toe, a location that has not previously been described, with underlying bone involvement noted on ultrasound and plain films.
A 64-year-old woman was presented with a 2-year history of an asymptomatic, expanding eruption on the chest, nape, and wrists (Figures 1 and 2). The patient reported a history of hypothyroidism without any other systemic symptoms. Histologic findings showed multiple macrophage infiltrates and multinucleated giant cells with elastolysis.Annular elastolytic giant cell granuloma is an entity of undetermined etiology that predominantly affects photoexposed areas. Sun protective measures and hydroxychloroquine are the most common therapeutic options. 1
