A febre por mordida de rato, também conhecida como Sodoku, é uma infecção bacteriana causada pela Spirillum minus e que tem como reservatório principal os ratos2. Apresenta distribuição universal, sendo frequente na Ásia, especialmente no Japão e é rara nos Estados Unidos.Clinicamente observa-se, no local da mordida, a presença de uma reação inflamatória caracterizada por uma celulite recoberta de vesículas, após um período de incubação de 1 a 4 semanas (média de 2 semanas). Esta lesão inicial (ponto de inoculação) cicatriza antes que se iniciem sinais ou sintomas, tais como: linfadenopatia regional, linfangite, cefaléia, náuseas, vômitos e febre alta. Artralgiae, mais raramente, erupção cutânea do tipo máculo-papular, especialmente em extremidades (região palmar e plantar) podem ser também encontrados. O ponto de inoculação inicialmente cicatriza para depois ulcerar. Cura espontânea pode ocorrer num período de 4 a 8 semanas3 4.O diagnóstico é predominante clínico e epidemiológico, entretanto a confirmação se faz através da identificação do Spirillum minus (pesquisa de campo escuro ou pela coloração de Wright).O tratamento de escolha é feito com penicilina, embora a eritromicina, estreptomicina, tetraciclina e clindamicina possam ser utilizadas1 4 9 . A mortalidade nos casos não-tratados varia de 10 a 13 %6.
