The aim of the presented work was to compare two methods of verification of the medical accelerator’s isocenter and determine the imaging offset. Two methods were used for verification - Machine Performence Check (MPC) application with the IsoCal phantom and the Artiscan software with the PTW Isoball Phantom. Each of the analyzed methods provides an alternative way to the time-consuming and costly verification of the isocenter on radiochromic or gafchromic dosimetry films. Measurements were performed for ten days and the obtained data were compared for both methods. Using statistical tests, concordance was determined for the results obtained by the two methods. The data were presented in trend function graphs. Analysis of the results shows that it is not possible to unequivocally state that the methods are not equivalent. In addition, the AAPM TG 142 report assumes that MPC is not a sufficient test for checking medical linear accelerator’s isocentre.
Jednym z najczęstszych nowotworów wśród mężczyzn w Polsce jest nowotwór gruczołu krokowego, a jednym ze sposobów jego leczenia jest radioterapia z jednoczasowym zintegrowanym boostem (SIB, ang. Simultaneous Integrated Boost), który umożliwia dostarczenie różnych dawek do różnych objętości docelowych w ramach jednej frakcji leczenia. Ze względu na położenie prostaty pomiędzy narządami krytycznymi niezbędne jest uzyskanie dużego spadku dawki wokół obszaru tarczowego (PTV, ang. Planning Target Volume) a jednym ze sposobów osiągnięcia tego jest zastosowanie w systemie planowania leczenia (TPS, ang. Treatment Planning System) Eclipse (Varian Medical Systems, Palo Alto, CA, USA) funkcji optymalizacji The Normal Tissue Objective (NTO). Jest to zoptymalizowany zestaw parametrów wejściowych, które ostatecznie pełnią funkcję pierścienia dookoła PTV. W pracy zostały porównane plany bez NTO, automatycznym ustawieniem NTO oraz o różnych kombinacjach priorytetu p (50, 200 i 500) oraz spadku dawki (ang. fall-off) k (0,05, 0,2 i 1). W planach zostały porównane rozkłady dawek, współczynniki konformalności (CI, ang. Conformity Index) i gradientu (GI, ang. Gradient Index) oraz dawki w narządach krytycznych (OAR, ang. Organs at Risk). Na rozkładach dawek wyraźnie widać, że NTO jest skutecznym narzędziem do ograniczenia dawki poza objętością PTV. Współczynnik konformalności określający stopień dopasowania kształtu izodoz do kształtu obszaru tarczowego i na wszystkich planach utrzymywał się na podobnym poziomie, a jego średnia wartość wynosiła 0,529±0,196. Współczynnik gradientu stanowi różnicę między promieniami odpowiadającymi izodozom dawki zadanej oraz połowy dawki zadanej, więc jego mniejsza wartość oznacza większy spadek dawki poza PTV. Obserwowany jest dla planów o większym priorytecie, ale niestety jest to związane ze spadkiem pokrycia w PTV. Analiza dawek w narządach krytycznych wykazała, że funkcja optymalizacji NTO wykorzystywana w planowaniu raka gruczołu krokowego, prawdopodobnie ze względu na jego anatomiczne położenie, nie ma istotnego wpływu na redukcję dawki w sąsiednich tkankach. Dalsze badania nad NTO zakładają większą grupę planów i wartości wejściowych parametru lub zmianę lokalizacji.
Radioterapia jest integralną częścią kompleksowego leczenia raka piersi, który stanowi ponad 20% wszystkich zachorowań na nowotwory wśród kobiet w Polsce. Wraz z postępem technologicznym, rozwijane są metody zarówno poprawiające skuteczność leczenia i komfort pacjentek jak i ograniczające toksyczność terapii. Najpopularniejszą techniką napromieniania w przypadku nowotworu piersi jest trójwymiarowa radioterapia konformalna (3DCRT, ang. Three-Dimensional Conformal Radiotherapy), gdyż zapewnia najniższe dawki promieniowania zaabsorbowane przez narządy kontrlateralne. Techniki dynamiczne, w porównaniu ze statycznymi, zapewniają w przypadku nowotworów piersi bardziej jednorodny rozkład dawki w objętości tarczowej (PTV, ang. Planning Target Volume), ale wyższe dawki w narządach krytycznych (OAR, ang. Organ at Risk). Coraz większą popularnością cieszy się metoda hybrydowa łącząca zalety planów statycznych i dynamicznych. Celem pracy było porównanie czterech technik napromieniania piersi: 3DCRT, obrotowej z modulacją intensywności dawki (VMAT, ang. volumetric modulated arc therapy), B-VMAT (ang. Butterfly VMAT) tzw. motyl, w którym emisja wiązki terapeutycznej w pewnym zakresie kątowym łuku jest zablokowana oraz techniki hybrydowej łączącej w jednym planie leczenia pola realizowane techniką 3D i VMAT. Dla grupy 11 pacjentek przygotowano w systemie planowania leczenia (TPS, ang. Treatment Planning System) Eclipse (Varian Medical Systems, Palo Alto, USA) i porównano plany leczenia dla każdej z omawianych technik. Oceniono rozkłady dawek, histogramy dawka-objętość (DVH, ang. Dose Volume Histogram) oraz dawki średnie w narządach krytycznych: płucach, płucu kontrlateralnym, sercu oraz zdrowej piersi. Na podstawie analizy DVH, stwierdzono, że wszystkie analizowane plany leczenia były klinicznie akceptowalne. Za najkorzystniejsze pod względem dawek deponowanych w OAR można uznać plany leczenia techniką 3DCRT. Jednocześnie należy zauważyć, że w technice hybrydowej dawki te są jedynie nieznacznie wyższe. Dla planów VMAT zaobserwowano zwiększenie procentowych objętości narządów krytycznych objętych izodozami niskich dawek, zwłaszcza dla narządów kontrlateralnych. Zablokowanie emisji wiązki w części łuku, tj. zastosowanie techniki B-VMAT częściowo zniwelowało ten efekt, jednakże w ograniczonym zakresie. Dla tzw. „motyli”, podobnie jak w przypadku standardowych planów VMAT, objętości OAR, w których deponowane są niskie dawki promieniowania są zauważalnie wyższe niż dla planów 3DCRT i hybrydowych. Hybrydowa technika napromieniania piersi stanowi dobrą alternatywę dla planów 3DCRT i może zostać wdrożona do zastosowania klinicznego, skracając czas realizacji planów leczenia na aparacie terapeutycznym i poprawiając tym samym komfort leczonych pacjentek.
