In Brazil, an increasing proportion of new HIV infections and AIDS cases involve women of reproductive age. To describe the reproductive desire of women with HIV/AIDS and to identify factors associated with the desire for motherhood, a cross-sectional study was carried out in the referral hospital for infectious diseases in Ceará State, northeast Brazil. In total, 229 women were included in data analysis. Median age was 32 years (interquartile range, 26-37), and 63% had a monthly family income of less than 210 USD. Forty-nine percent were using a contraceptive method, and 37% wished to undergo tubal ligation. Sixty-four percent of the latter women were motivated by the fear of having an HIV-positive child. Forty percent of the participants wanted to have a child. In the multivariate regression analysis, variables independently associated with women's desire to have a child were: younger age (in years, odds ration [OR] = 0.94; 95% confidence interval [CI]: 0.90-0.98), number of children (OR = 0.73; 95% CI: 0.57-0.96), and partner's desire for a child (OR = 3.35; 95%CI: 1.75-6.39). Having a partner who did not know about the woman's positive serostatus was negatively associated with the woman's desire for a child (OR = 0.17; 95% CI: 0.04-0.69). No variable related to clinical status was significantly associated with the outcome variable. Our data showed that many unsterilized HIV-positive women in northeast Brazil, at whatever stage of illness, have a desire for children. We recommend that nondirective counseling, consisting of helping women evaluate their own feelings, goals and needs with respect to reproductive options be provided.
Objectives: To assess and correlate the quality of life and adherence to antiretroviral therapy in people with HIV. Methods: A cross-sectional study was performed with 45 outpatients with HIV. The instruments used were: a questionnaire to assess adherence to antiretroviral therapy (CEAT-HIV), and a scale for assessing quality of life in people with HIV (HAT-QoL). A descriptive analysis was performed and the Spearman's linear correlation test was used. Results: Quality of life was compromised in the dimensions related to overall function; sexual function; health, disclosure and financial worries; and HIV mastery. Poor adherence prevailed in 51.3% of participants. The correlation between the scores of the scales was statistically significant in the dimensions of medication concerns and provider trust. Conclusion: The quality of life was compromised in six dimensions of the scale and adherence scale was inadequate in most people with HIV. ResumoObjetivos: Avaliar e correlacionar a qualidade de vida e a adesão à terapia antirretroviral em pessoas com HIV. Métodos: Estudo transversal, desenvolvido com 45 pessoas com HIV em tratamento ambulatorial. Os instrumentos utilizados foram: questionário para avaliação da adesão ao tratamento antirretroviral (CEAT-VIH) e escala para avaliação da qualidade de vida em pessoas com HIV (HAT-QoL). Foi realizada análise descritiva e empregado o teste de correlação linear de Spearman. Resultados: A qualidade de vida mostrou-se comprometida nos domínios relacionados às atividades gerais; sexuais; preocupação com a saúde, com o sigilo, com questões financeiras; e conscientização sobre o HIV. Houve predomínio da adesão inadequada em 51,3% dos participantes. A correlação entre os escores das escalas mostrou significância estatística nos domínios relacionados à medicação e confiança no profissional. Conclusão: A qualidade de vida apresentou comprometimento em seis domínios da escala e a adesão encontrou-se inadequada na maioria das pessoas com HIV.
Objetivou-se apreender os dilemas e conflitos revelados por mulheres que engravidaram na vigência da infecção pelo HIV/Aids. Estudo qualitativo com oito mulheres atendidas em ambulatório em Fortaleza-CE. Mediante entrevistas observou-se que as mulheres gestam com expectativa do resultado da sorologia do concepto; depois de nascidos vivenciam incertezas e mantêm superproteção da criança. Relatam maneiras inadequadas da comunicação do diagnóstico e pouca orientação para gestar em face do HIV. As mulheres transformam o desejo de ser mãe em realidade. Para elas, lidar com uma criança com possibilidade de contrair infecção pelo HIV é algo além de sua capacidade. Conclui-se ser essencial oferecer suporte emocional e social para essas mulheres independente das fases da vida.
IntroduçãoA assistência ao planejamento familiar é oferecida, atualmente, no Brasil, pelas equipes do Programa Saúde da Família (PSF), um modelo de política pública de saúde que traz a proposta do trabalho em equipe, de vinculação dos profissionais com a comunidade e de valorização e incentivo à participação comunitária. Corresponde a uma das sete áreas prioritárias de intervenção na atenção básica, definidas na Norma Operacional da Assistência 1 .O PSF tem o propósito de reverter a forma de oferta da assistência à saúde, ou seja, incorporando ações coletivas de cunho promocional e preventivo a substituir progressivamente o atendimento individualizado, curativo, de alto custo e de baixo impacto. Neste aspecto, é imprescindível o estabelecimento de parcerias intersetoriais com educação, ação social, trabalho, outras instâncias governamentais e a sociedade civil 2 . No Estado do Ceará, Brasil, o PSF teve implantação iniciada em 1994, oferecendo cobertura de 56% da população no período deste estudo, com equipes atuando nos seus 184 municípios 3 .As equipes do PSF são constituídas por um médico, um enfermeiro, um auxiliar de enfermagem e seis agentes comunitários de saúde (ACS), co-responsáveis pela saúde de cerca de mil famí-lias, o que corresponde à média de 3.450 a 4.500 pessoas 2 .ARTIGO ARTICLE
IntroduçãoA hipertensão arterial é considerada um dos fatores de risco mais importantes para o desenvolvimento de doenças cardiovasculares e sofre influência multifatorial, com destaque para o estilo de vida 1 .Conforme se indica, concomitante à terapêu-tica para o controle da hipertensão arterial, os indivíduos devem adotar um estilo de vida saudável, eliminando hábitos que constituam fatores de risco para a doença. Entretanto, no acompanhamento à saúde dos pacientes, segundo se observa, é grande a dificuldade para a aquisição de hábitos saudáveis, pois a tomada de decisão com vistas à superação de hábitos nocivos à saú-de, apesar de necessária, constitui uma decisão pessoal.Diante dessas polêmicas, é cada vez mais freqüente o interesse pelo assunto e a produção de pesquisas sobre avaliações de qualidade. A expressão qualidade de vida pode indicar diferentes aspectos do cotidiano do indivíduo ou de grupos específicos.De acordo com a definição da Organização Mundial da Saúde (OMS), qualidade de vida é a percepção do indivíduo sobre sua posição na vida, considerada no contexto da cultura e dos valores nos quais vive e elabora seus objetivos, expectativas, padrões e preocupações 2,3 . The objective of this study was to learn how women with Aids cope with daily living after receiving their diagnosis. It is an exploratory, descriptive study with a qualitative approach based on thematic oral history for data collection and analysis. Ten women with Aids were interviewed at a reference hospital in Fortaleza, State of Ceará, from December 2004 to March 2005. A semi-structured questionnaire was used for data collection. The results revealed different ways of coping, related to the fragility of the discovery of the diagnosis, to the need to hide the infection due to the discrimination and prejudice faced by the victims, and to the constant perception of death. In addition, the women expressed feelings such as shame, concern about family, abandonment, solitude, sadness, fear and anxiety. The authors conclude that, two decades after the epidemic broke out, women still face evident difficulties when they experience their infection.
OBJECTIVE: to analyze the quality of life (QoL) of men with AIDS from the perspective of the model of social determinants of health (MSDH). METHOD: cross-sectional study conducted in an outpatient infectious diseases clinic from a Brazilian university hospital over the course of one year with a sample of 138 patients. A form based on the MSDH was used to collect sociodemographic data addressing individual, proximal, intermediate determinants and the influence of social networks together with an instrument used to assess the QoL of people with HIV/AIDS. The project was approved by the Institutional Review Board (Protocol No. 040.06.12). RESULTS: according to MSDH, most men with AIDS were between 30 and 49 years old (68.1%), mixed race (59.4%), heterosexual (46.4%), single (64.5%), Catholic (68.8%), had a bachelor's degree (39.2%), had no children (61.6%), and had a formal job (71.0%). The perception of QoL in the physical, level of independence, environment, and spirituality domains was intermediate, while QoL was perceived to be superior in the domains of psychological and social relationship. A perception of lower QoL was presented by homosexual (p=0.037) and married men (p=0.077), and those with income below one times the minimum wage (p=0.042). A perception of greater QoL was presented by those without a religion (p=0.005), living with a partner (p=0.049), and those who had a formal job (p=0.045). CONCLUSION: social determinants influence the QoL of men with AIDS.
