VIPoma is a neuroendocrine tumor that secretes vasoactive intestinal polypectide (VIP) and produces a well-defined clinical syndrome characterized by watery diarrhea, hypokalemia, hypochlorhydria and metabolic acidosis. With low incidence, in 90% of cases the VIPoma arise from the pancreas. We present a new case of pancreatic VIPoma with successful resection and benign histology.
Los linfangiomas son un grupo poco frecuente de tumores benignos de los vasos linfáticos. Se localizan en su mayoría en tejido subcutáneo de cabeza, cuello y axila. Son raros en adultos y la afectación abdominal es infrecuente. Caso clínico Presentamos un nuevo caso de linfangioma quístico abdominal en una adolescente. Se trata de una joven de 17 años sin antecedentes personales de interés que acude a urgencias por dolor lumbar cólico izquierdo irradiado a hipocondrio izquierdo de 2 días de evolución. No presentaba náuseas ni vómitos, ni fiebre, ni síndrome miccional, ni alteración del hábito intestinal. En la exploración física el abdomen es blando, depresible, doloroso a la palpación en hemiabdomen izquierdo, donde parece palparse un efecto masa, sin defensa ni datos de peritonismo. Los análisis de sangre y orina son normales. La ecografía abdominal demuestra una gran masa quística en hipocondrio izquierdo con septos en su interior sugestiva de linfangioma. Se realiza tomografía computerizada (TC) abdominal evidenciándose la una masa quística de 9-10 cm, que se extiende por epigastrio e hipocondrio izquierdo, englobando el estómago y en contacto con el bazo y el lóbulo hepático izquierdo, compatible con linfangioma quístico (Fig. 1).
El angiosarcoma primario de bazo es un tumor poco frecuente, en torno a 200 casos descritos en la literatura mundial y con un mal pronóstico. Su presentación clínica y las imágenes radiológicas son bastante inespecíficas.Presentamos un nuevo caso de angiosarcoma primario de bazo que confirma de nuevo su agresividad. Se trata de un varón de 63 años con antecedentes de HTA y prostatismo que consulta por dolor abdominal epigástrico, en ocasiones irradiado a hipocondrio izquierdo, de 3 semanas de evolución, acompañado de sensación de plenitud no relacionada con la ingesta y adelgazamiento de 3 kg de peso.En la exploración física el abdomen es blando, depresible, ligeramente globuloso, con molestias a la palpación en hipocondrio izquierdo y esplenomegalia moderada.En la analítica se aprecia: hemoglobina 11 g/dl, hematocrito 34,1%, leucocitos 12.980 (63,5 % neutrófilos), 520.000 plaquetas. Coagulación: normal. Bioquímica: creatinina 1,4 mg/dl, resto de parámetros sin alteraciones. Marcadores tumorales: CA-19.9, CEA, PSA y alfafetoproteína normales. La tomografía axial computerizada (TAC) demuestra la existencia de una moderada esplenomegalia heterogénea con múltiples lesiones hipodensas compatibles con infarto y otras áreas nodulares sugestivas de patología infiltrativa, sin evidencia de adenopatías, y quiste hepático de 2 cm (Fig. 1). Se procede a biopsia espléni-ca guiada por TAC, informada como frotis casi exclusivamente hemático con escasa celularidad y aislada atipia nuclear no diagnóstica de malignidad.Se interviene de forma programada realizándose esplenectomía. El estudio histopatológico de la pieza quirúrgica demuestra que se trata de un angiosarcoma en un bazo de 658 g de peso y de 18 x 11 x 7 cm, con marcadores vasculares como el CD31 y CD34 intensamente positivos de forma difusa (Fig. 2).Al mes de la cirugía se le realiza gammagrafía ósea, en la que se aprecia incremento anómalo del trazador localizado en la quinta costilla derecha y el sacro sugestivo de afectación ósea metastá-sica, así como TAC abdominal donde se aprecian metástasis hepáticas. El paciente fallece a los 2 meses de la intervención.Descrito por primera vez por Langhans en 1879, el angiosarcoma primario de bazo es un tumor infrecuente que representa el 1-2% de los sarcomas. Su presentación clínica es inespecífi-ca y variable, siendo lo más frecuente el dolor abdominal, en ocasiones más localizado en hipocondrio izquierdo, la pérdida de peso, la anemia y la esplenomegalia (1-3). El hemoperitoneo por rotura esplénica espontánea ocurre alrededor del 30% de los casos y con frecuencia es la primera manifestación de la enfermedad (4).
