CyA action as a calcineurin inhibitor has allowed prolongation of kidney transplants, but its chronic use has led to devastating consequences such as allograft nephropathy. Previously, we have identified potential mechanisms of CyA-induced endothelial dysfunction in vitro. The current study identifies increased IL-6 expression as a mechanism by which CyA induces renal damage and that the use of an IL-6-neutralizing antibody may be useful in reducing CyA-induced renal damage.
Despite their anatomic proximity, communication between the colorectum and seminal vesicle is an uncommon event. We describe the first reported case of a fistula between the colon and seminal vesicle as a complication of diverticulitis. This case report is followed by a literature review of other reported cases of fistula formation between the seminal vesicle and the colon.
We report a case of a previously healthy 38-year-old male with acute prostatitis and concurrent Pseudomonas aeruginosa urosepsis. Pulsed-field gel electrophoresis analysis confirmed that the source of the organism was the patient's newly purchased hot tub, which was filled with water from a stream.
CASE REPORTA previously healthy 38-year-old white male presented to the emergency room with a 10-hour history of fevers, chills, and significant suprapubic pain. He also complained of dysuria and feelings of incomplete emptying after voiding. He denied any history of sexually transmitted diseases, recent urinary tract infections, or genitourinary trauma. He did mention having sexual relations with his wife three times the previous week in a newly purchased hot tub. His wife was asymptomatic.He received 6 liters of crystalloid fluid for hypotension in the emergency department. A Foley transurethral catheter was placed for urinary output measurement prior to urology consultation. A computed tomography scan of the abdomen and pelvis revealed significant soft tissue stranding surrounding the prostate, seminal vesicles, and posterior aspect of the bladder.
