La population des patients résidant en Ehpad (Établissement d'hébergement pour personnes âgées dépendantes) est de plus en plus vulnérable et fragile, et cette population doit pouvoir bénéficier d'un accès direct au service de court séjour gériatrique (filière « courte »). La facilité de cet accès (appel téléphonique du médecin traitant) ainsi que la « non-filtration » de ces patients par les urgences expose au risque d'admissions inadéquates. Ce travail a étudié la pertinence de 40 admissions directes de patients résidant en Ehpad dans un service de court séjour gériatrique. Au moyen de la grille AEPf, 82,5 % des admissions sont jugées pertinentes ou justifiées (52,5 % sont pertinentes au moyen de la grille AEPf, 30 % sont justifiées par le jury d'expert), 17,5 % des admissions sont jugées inappropriées. Certaines actions permettraient de diminuer les admissions inappropriées (mise en place d'équipes mobiles gériatriques et psycho-gériatriques intervenant dans les Ehpad, recours plus adapté aux structures ambulatoires, information des médecins de ville). Dans l'objectif de réduire l'intervention du jury d'experts, une adaptation de la grille AEPf aux patients gériatriques est proposée sous forme d'une grille « AEPg ». Au regard de la littérature, le processus d'admission directe ne semble pas majorer le taux d'admissions inappropriées.
