RESUMO Embora o risco de transmissão de doenças infecciosas para profissionais de saúde devido à exposição a material biológico seja conhecido, estima-se que grande parte dos acidentes seja subnotificada. Os objetivos desta pesquisa foram identificar a taxa e os motivos de subnotificação de acidentes ocupacionais ocorridos com profissionais de enfermagem do bloco cirúrgico de um hospital universitário. Assim, todos os profissionais de enfermagem do bloco cirúrgico que aceitaram participar da pesquisa foram entrevistados individualmente. Dos 161 participantes, 90 relataram ter sofrido um total de 261 exposições a material biológico das quais 147 foram subnotificadas, ou seja, uma taxa de subnotificação de 55,1%. Os principais motivos alegados foram: paciente-fonte HIV negativo e julgar que o acidente era de baixo risco. A implementação de estratégias para promover o aumento da segurança depende do conhecimento das situações que representam risco e da identificação dos fatores de risco individuais e institucionais.
