Introdução: O câncer de colo uterino é um importante problema de saúde pública, é o terceiro câncer mais frequente em mulheres e a quarta causa de mortalidade no Brasil. Dentre todos os tipos de câncer, o câncer do colo do útero é o que apresenta um dos mais altos potenciais de prevenção e cura, se diagnosticado e tratado precocemente. O exame de rastreio deste câncer é considerado umas das estratégicas públicas mais efetivas, seguras e de baixo custo para detecção precoce. O crescimento nos gastos para controle da doença acentua sua importância como problema de saúde pública, o tratamento do câncer do colo do útero representa um grande impacto na economia brasileira. Objetivos: Avaliar as características clínico-epidemiológicas e os custos das internações hospitalares e procedimentos realizados no SUS por câncer de colo uterino no período de 2012 a 2017. Métodos: Trata-se de um estudo transversal, descritivo, com informações coletadas a partir do banco de dados TabNet-DataSUS, utilizando o CID-10 "C53.0-Neoplasia maligna do colo uterino". Foram incluídas todas as faixas etárias que necessitaram de internação hospitalar, registrados no período de 2012 a 2017, em todos os Regiões brasileiras. Analisou-se os custos, caráter de internação e mortalidade. NO período do estudo a cobertura da vacina predominou no sexo feminino (86%), semelhante a estudos realizados nos EUA. A colpocitologia teve uma média de cobertura em torno de 28,55%. Em relação aos custos gastos com rastreamento e diagnóstico de câncer de colo uterino, obtivemos resultados inferiores em relação a outros países. A taxa média de internações hospitalares por ano foi de 3,54 internações/10.000 mulheres expostas. A idade média com maior número de internação foi 40 a 59 anos, com aproximadamente 6 dias de internação. Do ponto de vista econômico, o valor médio gasto por cada internação foi de R$ 1683,33. Os gastos com serviços hospitalares foram quatro vezes maiores do que o gasto com serviços de profissionais da área da saúde. Das pacientes internadas, 13.358 pacientes evoluíram para o óbito em 5 anos. Conclusão: Conclui-se, portanto, que ocorreu aumento significativo no número das internações hospitalares por câncer de colo uterino, e que a taxa de cobertura do exame colpocitológico está muito baixa, necessitando-se campanhas de estímulos à realização do mesmo.
Objetivos: Essa pesquisa tem como objetivo analisar o impacto da pandemia pela COVID-19 no acesso à saúde íntima da mulher na região Nordeste do Brasil. Métodos: Trata-se de um estudo observacional, transversal e descritivo, feito por meio de questionário online próprio, com mulheres nordestinas a partir dos 18 anos de idade. O cálculo amostral foi realizado através da equação derivada da estimativa de parâmetro populacional, encontrando um “n” mínimo de 385 pessoas. O formulário foi divulgado através das redes sociais, com a participação de 463 mulheres. Resultados: O perfil sociodemográfico da pesquisa foi composto majoritariamente por mulheres brancas (52,3%), de faixa etária entre 18-24 anos (47,5%) e que possuíam plano de saúde (78,4%). Sobre as 100 mulheres que não tinham acesso à saúde suplementar, 56% dependiam exclusivamente do SUS. Das 463 participantes, 63,8% se consultaram durante a pandemia, 33% não se consultaram e 3,2% marcaram a consulta, mas ainda não tinham ido. Dentre as que dependiam exclusivamente do SUS, 67,3% não foram atendidas, uma porcentagem aproximadamente duas vezes maior do que a da amostra geral. Quanto às mulheres que compareceram aos atendimentos, 44,5% enfrentaram alguma dificuldade de acesso à consulta, e 84,3% dessas limitações surgiram ou pioraram por conta da pandemia. Conclusão: Ainda que a maioria das participantes possua boas condições financeiras, um terço delas não se consultou durante a pandemia. Por diversos fatores, é notório que esse período impactou negativamente a saúde da mulher, reduzindo o número de consultas e aumentando as dificuldades de acesso.
Neuroendocrine tumor of the uterine cervix is a rare malignant neoplasm, ranging from 0.5 to 5% of uterine cervix cancers. Most of the cases occur in women younger (average 43 years) than those with the usual histologic types. It may occasionally be found associated with spindle cell carcinoma (SCC) and with adenocarcinoma (AC).The relationship between neuroendocrine tumor and human papillomavirus (HVP), specially the high-risk 16 and 18, is demonstrated in several studies. The diagnosis is definitive through the use of histologic hematoxyline-eosine staining, however identification of chromogranin, synaptophysin and enolase by immunohistochemistry plays an important role in the diagnostic doubt. The high tumor aggressiveness is demonstrated by the high percentage of lymph node impairment and early hematogenic invasion at the moment of diagnosis, 68% and 90%, respectively. Survival is short, with an average of 22 months. Multimodal treatment is the most frequently described form.
Objetivo: Analisar temporal e espacialmente as internações hospitalares por aborto no Brasil e regiões no período de 2012 a 2021. Metodologia: Estudo ecológico, com dados do Sistema de Informação Hospitalar (SIH/DATASUS) sobre internação hospitalar por aborto no Brasil e regiões de 2012 a 2021. Foram calculadas as taxas de internação hospitalar (TIH) e as Taxas Bayesianas Empíricas Espaciais (TBEE). As TIH foram utilizadas para análise de tendência temporal feita por meio do JoinPoint. As TBEE foram utilizadas para o modelo espacial, por intermédio do Índice de Moran (IM). Resultados: A tendência temporal do aborto por “outras gravidezes que terminam em aborto” foi de diminuição com Variação Percentual Anual – VPA de -0,7%, aberto espontâneo também foi de decréscimo com -3,9% e aborto por razões médicas foi de crescimento com 2,5% ao ano. O Norte foi a única região que apresentou aumento no aborto por outras gravidezes, com 2,6% ao ano, enquanto que por razões médicas o aumento foi de 8,9%. A região Sudeste apresentou predomínio de redução com -2,7% (outras gravidezes) e -4,5% (espontâneo) e Sul com -2,9% (espontâneo) e -3,9% (razões médicas). Na análise espacial foi identificado autocorrelação inversa entre o IDH e RPC e as TBEE de internação. As regiões Sul e Sudeste apresentaram predomínio de municípios com baixas taxas e altos IDH e RPC, enquanto Norte e Nordeste demonstrou altas taxas com baixos IDH e RPC. Conclusão: A tendência temporal foi diversa. O crescimento foi observado no aborto por razões médicas, principalmente no Norte, Nordeste e Centro-oeste. O Sudeste e Sul apresentaram redução. Houve correlação inversa entre os IDH e RPC dos municípios com as TBEE.
A Neoplasia Maligna do Colo de Útero (NMCU) tem alta incidência, é frequente nos países em desenvolvimento e é essencialmente causada pelo Papiloma Vírus Humano (99,7% dos casos). É o terceiro câncer mais comum entre mulheres (perde para mama e colorretal) e é a quarta causa de morte por câncer das brasileiras. Apesar da alta prevalência, a incidência mundial estimada, na faixa etária pediátrica, é de apenas 0,4 por milhão, e a brasileira, de 0,9 por milhão. O objetivo deste trabalho foi analisar os internamentos e óbitos por NMCU em faixa etária pediátrica no Nordeste entre 2018 e 2022. Para tanto, foi realizada coleta de dados na plataforma DATASUS sobre NMCU (CID C53.9) em pacientes pediátricos com as variáveis: faixa etária, ano, estado e etnia. Como resultado, obteve-se que 26.420 pacientes foram internados por NMCU, sendo 47 pacientes pediátricos, estes representam 0,17% dos casos no Nordeste. A taxa de letalidade na região foi a 2ª maior do país (6,38%). O Norte foi o 1° colocado, com 11,7%. A faixa etária mais acometida é de 15-19 anos, responsável por 82,9% dos internamentos e 66% dos óbitos. O ano de 2019 teve a maior quantidade de internações (40,42%) e os óbitos foram bem distribuídos. A cor mais afetada foi a parda, com 78,72% dos internamentos e 100% dos óbitos. Com isso, sabe-se que o Nordeste tem a 2ª maior taxa de letalidade pediátrica por NMCU do país e incidência aproximada da média mundial.
