Introdução: A tuberculose associada ao HIV modificou as perspectivas de controle da tuberculose, levando a um aumento da incidência da doença e sua morbimortalidade. Objetivo: Avaliar a epidemiologia e prevalência da coinfecção em um estado do nordeste brasileiro, bem como compará-lo com a situação geral do país e com dados encontrados na literatura. Metodologia: Trata-se de um estudo descritivo, de caráter quantitativo, da base de dados SINAN/DATASUS, dentre eles: pacientes com tuberculose a partir de variáveis como: sexo, HIV, ano de diagnóstico nos últimos cinco anos no estado de Alagoas e no Brasil. Resultados e discussão: O número total de pacientes diagnosticados com tuberculose no Estado de Alagoas foi de 5904 novos casos durante os anos de 2017 a 2021, sendo desses 634 (10.7%) possuem uma coinfecção do vírus HIV. Os alagoanos com tuberculose coinfectados com HIV correspondem a aproximadamente 2% do total nacional. A análise dos dados do SINAN em Alagoas mostrou convergência das informações com a literatura, revelando a prevalência do sexo masculino, com 2/3 do número de casos. possível justificativa para a redução importante do número de casos nos últimos dois anos se deve ao diagnóstico precoce e adequado, reforçado pelo Ministério da Saúde que recomenda que todos os pacientes ao receberem diagnóstico de tuberculose sejam rastreados para HIV, com início mais célere da Terapia Antirretroviral (TARV) e diminuição da morbimortalidade. Conclusão: Diante do cenário muito prevalente de coinfecção TB/HIV, torna-se necessário que novas políticas públicas surjam em favor da redução da desigualdade social, econômica e regional.
Introdução: Os Tumores do Estroma Gastrointestinal (GIST) ocorrem em 0,1-3% dos casos de tumores malignos do trato gastrointestinal que comumente surgem no estômago e exibem sintomas inespecíficos. De forma rara, esses tumores podem ser encontrados na topografia da adrenal ou podem mimetizar uma neoplasia adrenal, fato este pouco descrito na literatura. Relato do caso: O presente caso descreve uma paciente idosa diabética e tabagista, com histórico médico de nódulo adrenal há dois anos e sintomas inespecíficos, foi realizada tomografia computadorizada (TC) que evidenciou lesão heterogênea adrenal. Diante das imagens, do quadro clínico e dos exames laboratoriais, suspeitou-se de tumor adrenal não funcionante. Durante a cirurgia que se observou que o tumor na realidade se tratava de tumor na parede posterior gástrica sugestivo de GIST. Foi realizada gastrectomia parcial e o diagnóstico imunohistoquímico confirmou. Durante acompanhamento, paciente não evidenciou recorrência da doença. Conclusão: Apesar de incomum, este acontecimento evidencia necessidade de uma investigação diagnóstica e programação cirúrgica minusciosa principalmente nos pacientes oligossintomáticos.
