BACKGROUND: Odontogenic ameloblastoma (OA) of the jaws is a rare neoplasia of the oral cavity (0.78%) with a 5:1 mandible/maxilla relation, with the molar region and the ascendant ramus being the most affected areas. Comparing our results with the literature demonstrated to us that this tumor may be considered to be a worldwide problem due to the similarity in clinical findings among different ethnic groups CASE SERIES: The purpose of this study of 20 patients with ameloblastoma of the mandible and maxilla diagnosed at the Stomatology and Head and Neck Services of Heliópolis Hospital, São Paulo, Brazil, from 1980 to 1997, was to establish the histopathologic pattern of classification of those tumors through optical microscopy and the relation of this histopathology to therapeutic management. Using the Regeze, Kerr and Courtney classification (1978), we diagnosed follicular tumor in 11 cases, plexiform in 6 cases and unicystic in 3 cases, and performed surgical resection with a safety margin of 1.5 to 3.0 cm in the follicular and plexiform cases and bone curettage in the unicystic cases.
RESUMO: Objetivo: Avaliar a reatividade linfonodal em pacientes com metástase cervical de tumor primário oculto. Método: Foram avaliados retrospectivamente 24 pacientes submetidos a esvaziamento cervical entre 1983 e 1995 devido à metástase de tumor primário oculto. Os cortes histológicos dos 601 linfonodos resultantes foram corados pela hematoxilina-eosina e avaliados à microscopia óptica. A reatividade linfonodal considerou a presença de hiperplasia paracortical, hiperplasia de centro germinativo e hiperplasia sinusal. Foram avaliadas a relação da reatividade linfonodal, ruptura capsular, necrose e desmoplasia com a recidiva da doença. A análise estatística foi feita com Teste de Fisher com erro inferior a 5%. Resultados: Setenta e sete por cento (77%) dos linfonodos se mostraram reativos. Cinqüenta e cinco por cento (55%) dos pacientes com hiperplasia paracortical ou mista e dezesseis por cento (16%) dos pacientes com hiperplasia de centro germinativo ou linfonodos não reativos estavam assintomáticos por ocasião do último retorno ambulatorial (p=0,11). A presença de necrose tumoral mostrou associação significativa com a presença de desmoplasia (p=0,02). Conclusões: A reatividade linfonodal é freqüente na maioria das metástases cervicais em tumor primário oculto e a necrose tumoral está diretamente ligada à presença de desmoplasia. Methods: We perform a retrospective analysis of 24 patients submitted to neck dissection between 1983 and 1995 due to metastasis of occult primary tumor. Slices of 601 resulting lymph nodes was colored with hematoxilin-eosin and evaluated by optical microscopy. Lymph node reactivity patterns considered the presence of paracortical hyperplasia, germinal center hyperplasia and sinus hyperplasia. We evaluated relationship between lymph node reactivity, capsular rupture, necrosis and desmoplasia with relapse of disease. Statiscal analyses were performed through a Fisher Test with 2 error less than 5%. Results: Seventy seven percent of the lymph nodes were reactive. Fifty five percent of patients with paracortical or mixed hyperplasia and 16% of the patients with germinal center hiperplasia or non reactive lymph nodes were free of disease at the longest follow up (p=0,11). The presence of necrosis and desmoplasia showed a significant relation (p=0,02). Conclusions: The lymph node reactivity is usual in the great majority of primary unknown metastasis; tumoral necrosis is directly related with desmoplasia.
Introdução: Fazer a análise da presença da necrose tumoral microscópica no tumor primário inicial da laringe e correlacioná-la com algumas características clínicas e histopatológicas, objetivando identificar seu impacto na evolução. Forma de estudo: Retrospectivo clínico. Material e métodos: Estudo retrospectivo das fichas médicas e revisão dos cortes histológicos obtidos de 49 casos de carcinomas epidermóides da laringe, estadiados como T1 e T2, tratados no Serviço de Cirurgia de Cabeça e Pescoço do Complexo Hospitalar Heliópolis, de São Paulo/ SP, entre janeiro/1978 e dezembro/1997. Resultados: Houve forte associação entre a presença de necrose microscópica e a característica infiltrativa da lesão primária (p=0,004), lesões na supraglote (p=0,021), estádio clínico T2 (p=0,04), ocorrência de metástase cervical (p=0,04) e lesões menos diferenciadas (p=0,025). Aqueles casos que apresentaram necrose microscópica tenderam à melhor evolução. Conclusão: As informações obtidas do nosso estudo sugerem que a necrose por si, como classificada por técnicas histopatológicas, pode não ter influência exclusiva ou reflexo no crescimento volumétrico, refletindo a taxa de crescimento tumoral, mas pode estar relacionada a outros fatores tumorais e/ou do hospedeiro como a morte celular programada.
