In special risk situations such as heat waves, the risk/benefit ratio of psychotropic drugs which could interfere with body temperature regulation has to be carefully assessed, particularly in the elderly.
ResumenObjetivoEvaluar si el consumo de psicotrópicos se asocia con mayor riesgo de ingreso hospitalario por patologías relacionadas con el calor durante una ola de calor.MétodoRealizamos un estudio de casos y controles apareados. Los casos fueron las personas ingresadas en un servicio de urgencias por patología relacionada con el calor (hipertermia o golpe de calor) durante la ola de calor de agosto de 2003. Los controles fueron personas que vivían en la misma zona, pero que nofueron hospitalizados durante el mismo período, y que tenían, al menos, una prescripción enviada para ser reembolsada por la seguridad social en julio de 2003. Se usaron análisis multivariantes para identificar los psicotrópicos asociados por separado con la derivación al hospital durante el período de ola de calor.ResultadosEntre los 1.405 pacientes ingresados en el servicio de urgencias, 56 (4%) presentaban alguna patología relacionada con el calor. La edad media de los casos era de 83 años. Los análisis multivariantes demostraron que los casos tenían más probabilidades que los controles de estar siendo tratados con anticolinérgicos (CP 6, IC 95% 1,8-19,6), antipsicóticos (CP 4,6, IC 95% 1,9-11,2) o ansiolíticos (CP 2,4, IC 95% 1,3-4,4).ConclusiónEn situaciones especiales de riesgo, como las olas de calor, debe evaluarse cuidadosamente el cociente riesgo/beneficio de los psicotrópicos, ya que que podrían interferir con la regulación de la temperatura corporal, en particular, en ancianos.
