Résumé Une fragilité psychique profonde préexiste à toute alcoolisation abusive et itérative ultérieure. C’est pourquoi une recherche en clinique projective d’inspiration psychanalytique est actuellement menée sous l’égide de l’IREB (Institut de Recherche et d’Etudes scientifiques sur les Boissons), pour éclairer l’économie psychique de patients alcooliques graves. 18 protocoles de Rorschach ont pu être recueillis dans divers lieux de soins (9 hommes et 9 femmes exempts de pathologies organiques avérées et âgés de 33 à 69 ans, moyenne 49 ans). Un tel recueil permet une exploration clinique qualitative d’une « pathologie des confins ». En deçà de représentations de soi problématiques, il a été possible de mettre aussi au jour certaines malformations des représentations corporelles, en raison de fixations précoces qui appelleraient ultérieurement ce lien mortifère à l’alcool. L’émergence d’un tel langage corporel implique des aménagements dans les prises en charges psychothérapiques.
