Las masas en cuello constituyen un motivo de consulta muy frecuente en la práctica de medicina general. Pueden ser la única sintomatología que presente un paciente adulto con una neoplasia ya sea maligna o benigna de la región cervical. La etiología infecciosa constituye una causa muy frecuente; sin embargo, se debe siempre descartar malignidad en pacientes que presenten factores de riesgo para desarrollar cáncer de cabeza y cuello. Los estudios complementarios deben realizarse de manera individualizada según el paciente a tratar. La referencia al especialista debe hacerse de forma precoz así como el tratamiento específico para mejorar el pronóstico del paciente.
Los craneofaringiomas son tumores benignos intracraneales de origen epitelial poco frecuentes. Se caracterizan por infiltrar estructuras adyacentes como el hipotálamo, la glándula hipofisiaria y el quiasma óptico. Existen dos estirpes histológicas, la adamantinomatosa y la de tipo papilar, la cual se encuentra casi exclusivamente en población adulta. Pese a que sus manifestaciones son inespecíficas, la sospecha diagnóstica se basa en hallazgos clínicos y técnicas de imagen. Sin embargo, el diagnóstico definitivo se realiza mediante estudios histopatológicos. El craneofaringioma se considera la neoplasia selar más frecuente en la población pediátrica y puede llegar a impactar negativamente su crecimiento y desarrollo. Aunque es una patología con excelente supervivencia, genera una alta morbilidad en los pacientes, tanto antes como después de su tratamiento. Por este motivo, los más recientes avances en el tratamiento del craneofaringioma se han enfocado en minimizar los efectos adversos y las morbilidades secundarias a este.
La hipoacusia es la deficiencia sensorial más frecuente en la infancia. Es una condición incapacitante que afecta tanto al paciente como a su familia; no obstante, gracias al tamizaje auditivo neonatal y la detección temprana, el impacto sobre el desarrollo del lenguaje y la comunicación puede ser mínimo. Los síndromes con afectación auditiva, las infecciones intraútero y las infecciones postnatales son algunas de las causas de sordera en niños. Las causas genéticas no sindrómicas son responsables de la mayoría de los casos. El diagnóstico definitivo de la enfermedad se realiza mediante la aplicación de pruebas audiológicas subjetivas y objetivas. El tratamiento debe se instaura de forma precoz, de manera multidisciplinaria y depende específicamente de la etiología.
El hiperparatiroidismo primario representa el tercer padecimiento endocrinológico más frecuente en la actualidad. La mayoría de los casos cursa de manera asintomática, por lo que es una patología infradiagnosticada. Existen tres etiologías principales de hiperparatiroidismo primario: el adenoma, la hiperplasia y el carcinoma paratiroideo. Los métodos diagnósticos van desde estudios de laboratorio hasta métodos de imagen como ultrasonido, cintigrama, tomografía axial computarizada y resonancia magnética. El tratamiento de estas entidades se basa principalmente en la resección de las glándulas paratiroideas.
El bocio se define como la hipertrofia del tejido tiroideo. En ocasiones, puede extenderse hacia la cavidad torácica, comprometiendo estructuras mediastinales como la vena cava superior y el NLR. La disnea, la disfagia y la tos constituyen los síntomas más característicos de los pacientes con bocio intratorácico. Debido a su crecimiento lento y progresivo, esta patología podría pasar desapercibida clínicamente; no obstante, su diagnóstico se realiza con facilidad por medio de imágenes. Estas permiten no solamente valorar el tamaño, extensión de la masa y su relación con estructuras adyacentes, sino también planear el abordaje quirúrgico, que constituye el tratamiento definitivo en estos casos.
