A esclerose sistêmica (ES) é uma patologia crônica e autoimune do tecido conjuntivo, que causa perda da mobilidade, da funcionalidade, da amplitude de movimento, da força muscular e do condicionamento cardiopulmonar, e prejuízo na qualidade de vida. Este estudo objetivou verificar os efeitos de um programa de intervenção fisioterapêutica na força de preensão palmar e na qualidade de vida de um idoso longevo portador de ES. Trata-se de um estudo de caso, longitudinal e intervencionista. O paciente foi atendido no Serviço de Fisioterapia Reumatológica da Universidade de Passo Fundo e o protocolo de intervenção foi baseado em um programa de intervenção cinesioterapêutica. Para a mensuração da força de preensão palmar foi utilizado o Dinamômetro Kratos® e, para avaliação da qualidade de vida utilizou-se o SF-36. Os resultados demonstraram melhora da força de preensão palmar bilateral quando se compara os resultados das avaliações pré e pós intervenção, especialmente no membro esquerdo. No que diz respeito à qualidade de vida, observou-se melhora geral nos domínios do questionário SF-36. A cinesioterapia mostrou-se um método eficaz na melhora da força de preensão palmar e da qualidade de vida de um idoso longevo com ES.
BACKGROUND AND OBJECTIVES: Rheumatoid arthritis is an inflammatory, chronic and progressive disease. It impairs joint synovial membranes and may induce bone and cartilage destruction. Many diseases may follow rheumatoid arthritis, including systemic lupus erythematosus, an inflammatory, chronic autoimmune disease with multisystemic manifestations, with periods of remission and exacerbation. This study aimed at reporting kinesiotherapy intervention for quality of life, pain and muscle strength of a patient with rheumatoid arthritis and systemic lupus erythematosus. CASE REPORT: Female patient, 49 years old, diagnosed 15 years ago with rheumatoid arthritis and systemic lupus erythematosus. Patient complained of pain on hands, feet and lumbar spine, with irradiation to lower limb and morning stiffness. Tool to measure muscle function was Biodex System3 Pro isokinetic dynamometer in the speeds of 120 and 240° during knee flexion and extension movements, in addition to the Short-Form Health Survey questionnaire and pain evaluation by the visual analog scale. CONCLUSION: The study has shown that kinesiotherapybased physiotherapy is effective to relieve pain and improve muscle strength and quality of life of patient with systemic lupus erythematosus and rheumatoid arthritis.
ResumoAs alterações decorrentes do processo de envelhecimento, evidenciadas pela diminuição da força muscular, da amplitude de movimento, da velocidade de contração muscular, da acuidade visual e auditiva e pelas alterações posturais influenciam a mobilidade funcional e o equilíbrio em idosos. Essas alterações podem acarretar alguns prejuízos para o idoso, como o aumento do risco de quedas e a redução do nível de independência funcional. A amostra foi composta por 76 idosos, a maioria do gênero feminino, na faixa etária de 60 a 69 anos, casada, com baixas escolaridade e renda. Os indivíduos portam alguma doença associada, sendo a hipertensão arterial sistêmica a mais relatada, fazem uso de medicamentos e de óculos para correção de problemas visuais. A maior parte dos idosos que sofreu quedas nos últimos seis meses apresentava um ou mais fatores de risco para quedas, evidenciando a probabilidade de que o uso de medicamento, a presença de fatores de risco cardiovascular e alterações visuais podem aumentar, efetivamente, o risco de quedas nessa população.
