Se presenta el caso de una paciente femenina de 65 años, previamente conocida sana, con un cuadro de dolor abdominal persistente en quien se diagnostica por medio de ultrasonido un quiste hepático simple gigante, el cual se abordó por vía laparoscópica.
La fotosensibilidad inducida por fármacos hace referencia a las erupciones cutáneas producto de una respuesta exagerada o anormal de la piel posterior a la exposición a radiación ultravioleta (UV), ya sea de la luz solar o de una fuente artificial en pacientes que se encuentran utilizando algún medicamento. Estas pueden subdividirse en reacciones fototóxicas y fotoalérgicas. La reacción de fototoxicidad es dada por una respuesta no inmunológica que resulta en lesión celular y daño tisular, mientras que la fotoalérgica corresponde a una reacción inmunológica de hipersensibilidad tipo IV tardía. Los medicamentos más comúnmente asociados a estas reacciones incluyen la amiodarona, la clorpromazina, la doxiciclina, la hidroclorotiazida, el naproxeno, el piroxicam, las tetraciclinas, el voriconazol, la griseofulvina, las quinolonas, las sulfonamidas, los antinflamatorios no esteroideos (AINE) y los retinoides sistémicos. La presentación de la reacción dependerá de la dosis de los medicamentos, así como del tiempo de exposición a la radiación UV. El diagnóstico se basa principalmente en una historia clínica detallada y la exploración física, aunque varios métodos como el fototest o el fotoparche pueden ser útiles. La mayoría pueden tratarse con compresas frías, emolientes o calamina tópica, sin embargo, dependiendo del caso se pueden llegar a necesitar analgésicos orales y corticosteroides tópicos. Si la afectación es grave se pueden utilizar corticosteroides sistémicos.
La vancomicina es un antimicrobiano de uso frecuente a nivel intrahospitalario para el tratamiento de infecciones graves por Staphylococcus aureus meticilino resistente, entre sus efectos secundarios de mayor cuidado se encuentra la nefrotoxicidad. Esta, en presencia de otros factores de riesgo propios del paciente, predisponen a la aparición de la lesión renal aguda.
Los riñones son de los órganos que sufren más cambios durante el envejecimiento. Existe una serie de mecanismos involucrados en el proceso de senescencia renal que explican los cambios estructurales, funcionales y moleculares que suceden intrínsecamente en este órgano. Uno de los más estudiados es el gen Klotho, cuya disminución favorece que los procesos dañinos conlleven a arteriosclerosis y la progresión de daño renal permanente. Otros mecanismos detallados en esta revisión incluyen el factor de crecimiento fibroblástico 23, la senescencia celular, el acortamiento de telómeros, la inflamación crónica y la señalización Wnt que suele estar sobreexpresada durante el envejecimiento.Comprender estos mecanismos favorecerá a futuro la implementación de diferentes intervenciones para detener o enlentecer la fibrosis y esclerosis que se desarrolla en los adultos mayores conforme aumenta la edad.
