We present a case of duodenal malrotation associated with microgastria, inverted splenic rotation and retrogastric spleen, treated succesfully by a Ladd´s procedure, hiatal reconstruction and partial fundoplication (Rev. Col. Bras. Cir. 2004; 31(5): 338-339) -ISSN 0100-6991.
RESUMO: Objetivo: Relatar a experiência no diagnóstico e tratamento do cisto de colédoco no Hospital Municipal Jesus em cinco anos (1996)(1997)(1998)(1999)(2000)(2001)) e a experiência brasileira publicada para o tratamento do cisto de colédoco em crianças. Método: Avaliação prospectiva dos pacientes operados com o diagnóstico de cisto de colédoco em cinco anos no Hospital Municipal Jesus. Resultados: Houve predomínio de casos diagnosticados pela presença de dor abdominal recorrente, icterícia e/ou massa abdominal palpável. Apenas dois pacientes apresentavam a tríade completa. Todos os pacientes foram submetidos à ultra-sonografia e houve dúvida no diagnóstico ultrasonográfico em apenas dois. Todos eram cistos do tipo 1 de Todani tratados através de ressecção completa e anastomose biliodigestiva em Y Roux. Houve complicações pós-operatórias imediatas em três pacientes: um caso de hemorragia digestiva alta e dois casos de fístula biliar, um deles relacionado a problemas técnicos na anastomose que necessitou de reoperação. O seguimento variou de quatro meses a cinco anos, sem detecção de complicação tardia em nenhum caso. Conclusões: O tratamento do cisto de colédoco através da ressecção completa na criança é seguro, relacionado a poucas complicações e capaz de resolver por completo a sintomatologia pré-operatória dos pacientes.Descritores: Cisto de colédoco; Malformações de vias biliares; Hepático-jejunostomia. 336 INTRODUÇÃO INTRODUÇÃO INTRODUÇÃO INTRODUÇÃO INTRODUÇÃOOs cistos de colédoco são típicos do paciente pediá-trico e apesar de incomuns na população ocidental têm sido progressivamente mais diagnosticados, pelo instrumental diagnóstico aperfeiçoado e uso difundido da ultrasonografia, capaz de detectar a doença com facilidade, inclusive in utero.Paralelamente ao aperfeiçoamento dos métodos diagnósticos ocorreu mudança radical do ponto de vista do tratamento cirúrgico: pelo reconhecimento relativamente recente (em torno de 20 anos) do alto risco de malignização tardia no epitélio dos cistos o tratamento transitou da aceitação de anastomoses cistoentéricas, para a imposição de ressecções completas do cisto ou, ao menos, de sua mucosa. A transição de uma cirurgia relativamente simples de executar (cistojejunostomia) para uma técnica mais complexa (ressecção completa do cisto com anastomose hepaticojejunal em Y Roux) e o diagnóstico e tratamento mais precoces obriga a uma revisão dos resultados terapêuticos, a fim de verificar a aceitação, praticidade e segurança dos métodos de ressecção em nosso meio. Resumo Objetivo: estudar as manifestações clínicas e radiológicas e o tratamento cirúrgico das duplicações pieloureterais sintomáticas na criança, especialmente com relação ao resultado das heminefrecto-mias polares superiores. Material e métodos: estudo prospectivo de observação num grupo de 15 pacientes pediátricos, tratados cirurgicamente, segundo um protocolo, com seguimento mínimo de 6 meses pós-operatórios. Resultados: 93% dos pacientes se apresentaram, no primeiro ano de vida, por infecção urinária e/ou hidronefrose fetal. Hemine-frectomia polar superior foi a cirurgia mais freqüente (80%). Todos os pacientes obtiveram melhora sintomática e radiológica após o procedimento, que não apresentou complicações significativas. As três crianças portadoras de refluxo vesicoureteral grave para o segmento superior da duplicação apresentaram refluxo sintomático para o coto ureteral no pós-operatório e sofreram ureterectomia distal. Nenhuma das crianças sem refluxo vesicoureteral pré-opera-tório necessitou de um segundo tempo cirúrgico. Conclusões: a heminefrectomia polar superior é um método seguro e eficaz para o tratamento das duplicações pieloureterais sintomáticas da criança. Em pacientes que não apresentam refluxo vesicoureteral associado, provavelmente será a única cirurgia neces-sária. No caso de pacientes portadores de refluxo vesicoureteral severo para o pólo superior de duplicação, poderá ser necessária ureterectomia distal posteriormente. Methods: prospective observational study concerning 15 pediatric patients surgically treated following a protocol with a minimum follow-up of 6 months. Results: 93% of the patients presented urinary infection and/or fetal hydronephrosis during their first year of life. These were the usual reasons for evaluation. Superior polar heminephrectomy was indicated for 80% of the patients, all of whom presented clinical and radiological improvement after the surgery. There were no significant complications. All children with severe vesicoureteral reflux to the duplicated superior pole presented symptomatic reflux to the stump after the surgery and were submitted to stumpectomy. None of the patients without preoperative reflux needed any other surgical procedure. Conclusions: superior polar heminephrectomy is safe and efficient in order to treat ureteral duplications in childhood. If the patient does not present vesicoureteral reflux this will probably be the only surgery needed. Most patients with reflux to the resected superior pole will need stumpectomy subsequently.
