Descriptive study of a population of COPD patients assisted at the Pasteur Hospital: severity and impact on daily life Estudo descritivo de uma população de pacientes com DPOC atendidos no Hospital Pasteur: gravidade e impacto na vida diária Resumen: Introducción. La EPOC representa un importante problema sanitario. En Uruguay la prevalencia es de 19.7%. Se caracteriza por limitación persistente del flujo aéreo asociada a repuesta inflamatoria crónica de las vías aéreas y destrucción del parénquima pulmonar frente a partículas nocivas. Objetivos: Conocer las características clínico-demográficas de una población de pacientes EPOC. Determinar la severidad de la enfermedad y su impacto en la vida diaria. Materiales y métodos: Estudio observacional, descriptivo y transversal de 61 pacientes captados al ingreso en el servicio de Medicina Interna del Hospital Pasteur y seguimiento en la policlínica de EPOC entre el 1° de octubre de 2015 y el 31 de diciembre de 2016. Los criterios de inclusión fueron: ambos sexos, mayores de 40 años, clínica y factores de riesgo compatibles o diagnóstico previo de EPOC. Los criterios de exclusión fueron: deterioro cognitivo moderado-severo, pacientes postrados, embarazadas, tuberculosis pulmonar activa, enfermedades neuromusculares. El muestreo se realizó a conveniencia. Los datos se obtuvieron del seguimiento ambulatorio. El procesamiento de los datos se hizo en el programa EPI-DATA versión 3.1 y PSPP. Resultados: De los 61 pacientes captados, se confirmó el diagnóstico de EPOC por espirometría en 42. La edad media de los EPOC fue 63 años (± 11), predominando en el sexo masculino (24, 57.1%). La mayoría tenía un bajo nivel de instrucción (primaria incompleta en 15 pacientes, 35.7%). El tabaquismo estuvo presente como factor de riesgo en el 100%, siendo el IPA promedio 81.9 (± 59.8). Más de la mitad asociaron al menos 2 factores de riesgo. 21 pacientes (50%) tenían disnea grado 3 del mMRC, mientras que 26 (61.9%) eran exacerbadores frecuentes. El valor medio obtenido en el cuestionario CAT fue 19.58 (DE 10.56). Por ende, la mayoría pertenecía a los grupos D (17, 40.4%) y C (9, 21.4%) definidos por las Guías GOLD 2017. En cuanto a la limitación al flujo aéreo, gran parte correspondió al Grupo GOLD 2 o limitación moderada. Conclusiones: La existencia de sesgos de selección determinó un pequeño de pacientes EPOC. La edad media y el sexo concordaron con datos internacionales. El bajo nivel de instrucción puede tener implicancias en la adherencia a controles y tratamiento. El IPA promedio fue más elevado que en estudios internacionales. La asociación de factores de riesgo se vinculó a peores resultados. Los pacientes EPOC analizados eran muy sintomáticos, presentaron exacerbaciones frecuentes y tenían un importante impacto en la calidad de vida, a diferencia de lo observado en grandes estudios europeos que evidencian un mejor control sintomático de los EPOC incluidos. Lo anterior no se correlacionó con una limitación severa al flujo aéreo, tal como lo señala la literatura internacional.
Zinc borates are very important industrial materials with applications that include fire retardants and preservatives in wood. In this work, we report the preparation of three novel zinc borates: [Zn(NH)BO(OH)]·HO (ZB1), Zn(HBO)·2NH·4HO (ZB2), and [Zn(NH)][BO(OH)]·4HO (ZB3). The solid phases were characterized by elemental analysis, X-ray diffraction, infrared and Raman spectroscopy, scanning electron microscopy and thermal analysis. The crystal structures of ZB1 (monoclinic, Cc, a = 12.1972(8), b = 7.8314(5), c = 12.1441(8) Å and β = 107.404(5)°) and ZB3 (orthorhombic, Pbca, a = 15.0796(9), b = 11.8853(5) and c = 16.7606(8) Å) were determined. They are novel neutral Zn-polyborate complexes [Zn(NH)BO(OH)] hydrate and [Zn(NH)][BO(OH)] tetrahydrate salt, respectively. The complete assignment of infrared and Raman spectra was performed theoretically using DFT calculations. For ZB2 (obtained as a polycrystalline phase), powder X-ray diffraction confirmed a single phase and allowed the determination of the unit cell parameters and lattice type (rhombohedral, a = 36.78076 (6), c = 12.20052 (3) Å) with an expected formula Zn(HBO)·2NH·4HO suggesting that this compound is a complex triborate. Furthermore, a low-cost scalable synthetic procedure for ZB1 starting from zinc oxide and boric acid in ammonia solution with a high yield is also reported.
