Methods: A review of the literature was carried out up to July 2012, with the inclusion of additional articles as far as August 2013, all of which were analyzed through the Oxford Centre for Evidence-Based Medicine (OCEBM) system. Results: 1. There is greater SIBO probability in IBS when breath tests are performed, but prevalence varies widely (2-84%). 2. The gut microbiota in individuals with IBS is different from that in healthy subjects, but a common characteristic present in all the patients has not been established. 3. The incidence and prevalence of PI-IBS varies from 9-10% and 3-17%, respectively, and the latter decreases over time. Bacterial etiology is the most frequent but post-viral and parasitic cases have been reported. 4. A sub-group of patients has increased enterochromaffin cells, intraepithelial lymphocytes, and mast cells in the intestinal mucosa, but no differences between PI-IBS and non PI-IBS have been determined. 5. Methanogenic microbiota has been associated with IBS with constipation. 6. Rifaximin at doses of 400 mg TID/10 days or 550 mg TID/14 days is an effective treatment for the majority of overall symptoms and abdominal bloating in IBS. Retreatment effectiveness appears to be similar to that of the first cycle.Conclusions: Further studies are required to determine the nature of the gut microbiota in IBS and the differences in low-grade inflammation between PI-IBS and non PI-IBS. Agradecemos a García-Mazcorro et al. 1 su interés en nuestra revisión 2 y nos permitimos enviar nuestra respuesta. En primer lugar, debemos anotar que estamos totalmente de acuerdo con sus comentarios respecto al término «flora»intestinal. El término correcto es microbiota, definida como la comunidad de microorganismos vivos residentes en un nicho ecológico determinado. Sin embargo, hay que anotar que microbiota no solo es la palabra con la cual inicia el título del artículo, sino que se menciona 79 veces a lo largo del mismo, mientras que flora solo se mencionó 5 veces 2 .Segundo, el gen 16S ribosomal (rARN) es uno de los componentes de la subunidad pequeña (30S) del ribosoma y se encuentra presente en todas las bacterias y archaea. Es el marcador genético que se utiliza en el análisis filogenético bacteriano y es ampliamente utilizado en un sinnúmero de estudios. Este gen es de aproximadamente 1,550 bp y está compuesto de 9 regiones de alta variabilidad o regiones hipervariables flanqueadas por regiones estables. Las diferencias en la secuenciación de estas regiones hipervariables permiten la identificación taxonómica de las bacterias presentes en las muestras de estudio (p. ej., heces, mucosa intestino) 3 . El gen «rrs», también denominado ADN ribosomal 16S o 16S rADN, como se menciona en la tabla 3 de nuestro artículo, codifica el 16S rARN. Si bien existen limitantes en la secuenciación basada en el 16S rARN, esta continúa siendo el estándar de oro debido a lo extenso de las bases de datos basadas en este marcador. El Illumina hace parte de las tecnologías de secuenciación de la «próxima generación» (next generation) que están limitadas por la longitud de las secuencias que pueden proveer y, por lo tanto, se deben seleccionar regiones específicas del 16S rARN en el análisis 3 . Existen en la actualidad algunas otras estrategias y equipos de secuenciación que se diseñaron para la complementación y corrección.En tercer lugar, la incapacidad de establecer a ciencia cierta la microbiota propia del SII por los factores mencionados por García-Mazcorro et al. es un aspecto fundamental de nuestra revisión 2 . De hecho, por ello hemos concluido que si bien las evidencias muestran que en SII la microbiota intestinal es diferente de la de sujetos normales (nivel de evidencia 3 b, recomendación grado B), no es posible establecer una composición microbiana propia de este trastorno (nivel de evidencia 3 b, recomendación grado B) 2 .Por otro lado, estamos de acuerdo en que en algunos casos no se especifican los niveles taxonómicos encontrados, pero también hay que anotar que algunos de los estudios revisados solo reportaron las diferencias en grupos microbianos. Finalmente, estamos de acuerdo con que nuestra ya extensa revisión, se queda corta en cuanto a que no analiza el uso de probióticos, prebióticos, incluso simbióticos en el tratamiento del SII, pero consideramos que estos temas serían objeto de otro documento.
