Utilità clinica del QuantiFERON TB Gold Test nella diagnosi di infezione tubercolare latente SUMMARY: Introduction: Early diagnosis of latent tuberculosis infection can avoid a progression to active tuberculosis.The interferon gamma (IFN-γ) production measured by the new interferon-gamma release assays (IGRAs) can be used to identify subjects with, because the tuberculin skin test, is unreliable to discriminative between subjects with and others. Methods: 186 patients tested positive with the tuberculin skin test were evaluated with QuantiFERON-TB Gold [QFT-G (Cellestis Ltd, Carnegie, Australia)]. Results: 61 (32.8%) samples showed a positive response with QFT-G test and only 1 (0.54%) patient tested indeterminate. Conclusion: These preliminary data suggest that QFT-G-positivity showed a strong correlation with clinical suspect of latent tuberculosis infection.

Alcuni studi molecolari sul ruolo del virus di Epstein-Barr (EBV) nell'eziopatogenesi dei CTCL, hanno portato a risultati controversi anche a causa della diversa sensibilità delle tecniche impiegate. Metodi. In questo lavoro è stata indagata retrospettivamente la presenza e l'eventuale carica virale di EBV in biopsie cutanee di pazienti con MF e SS utilizzando una PCR quantitativa-competitiva (QC-PCR) altamente sensibile (1-10 copie di EBV-DNA/reazione) messa a punto nel Laboratorio di Virologia. Tale metodica prevede la costruzione di una curva di taratura mediante l'utilizzo di uno standard esterno, un controllo interno di amplificazione e l'analisi densitometrica delle bande di amplificazione. Sono stati utilizzati campioni congelati di DNA di 17 pazienti con MF (11 M, 6 F; età mediana 63 anni, range 14-84), 4 con MF evoluta in linfoma ad alto grado (2 M, 2 F, età mediana 74 anni, range 59-82), 10 con SS (3 M, 7 F, età mediana 72 anni, range 46-84). Tutti i casi di MF e SS presentavano un riarrangiamento clonale dei geni del T Cell Receptor (TCR) catena γ. Come controllo sono stati esaminati i campioni di 8 pazienti con dermatosi cutanee reattive (4 M, 4 F, età mediana 64,5, range 33-79). Risultati. Nessuno dei 21 pazienti con MF è risultato positivo per EBV-DNA. Nella SS, invece, erano positivi per EBV/DNA 7 pazienti su 10 (70%) con una carica media di 313,4 copie genomiche/µg di DNA estratto (range 1-2160 copie). In nessun paziente del gruppo di controllo è stata riscontrata la presenza di EBV-DNA. Conclusioni. Nella nostra casistica appare una differenza altamente significativa tra MF e SS per quanto riguarda la positività per EBV-DNA nelle biopsie cutanee. Le differenze con i dati della letteratura, soprattutto per la MF, sono da valutare dal punto di vista delle tecniche impiegate. INTRODUZIONELa malattia da graffio di gatto o Cat Scratch Disease (CSD) è un'infezione sostenuta da microrganismi del genere Bartonella (B. henselae, B. quintana, B. helizabethae e bacilliformis), trasmessi all'uomo attraverso lesioni provocate dal graffio dell'animale domestico. L'agente infettante è un bacillo Gram-negativo, aerobio obbligato, non ossidante e non fermentante (inerte), classificato precedentemente nel genere Rickettsie dal quale è stato successivamente separato in quanto non richiede, come le Rickettsie, substrati cellulari, ma può essere coltivato in vitro su idonei terreni arricchiti con sangue (umano, di coniglio, di cavallo); inoltre, studi effettuati sul DNA e sull'RNA ribosomiale 16S non hanno evidenziato alcuna correlazione tra i due generi. Il reservoir dei microrganismi è il gatto domestico e la trasmissione avviene attraverso il graffio o il morso dell'animale. This work is a comparative clinical trial about the performance of a new automatic system RAD 120, with final fluorescent determination, and ALISEI, a referential system that works in classical ELISA.This comparison show that RAD120 has some hardware and software problems, that determinate false positive dosages, especially in IgM of Toxoplasmosis and Cytomegalovirus. This system is in evaluation too after adjournment, and only after this second test, we could be sure if RAD 120 can be became another referential system in torch diagnosis.
