Introdução: Com a epidemia de obesidade, as cirurgias bariátricas tornaram-se mais realizadas e as complicações pós-operatórias, como a trombose da veia porta mesentérica (TPM), mais comuns. Relato do caso: Relatamos um caso de TPM no pós-operatório de gastrectomia vertical. Conclusão: Diante uma sintomatologia inespecífica junto à manifestação tardia da TPM, faz-se necessário o conhecimento desta, para investigação adequada no pré e pós-operatório da gastrectomia. O bócio é o aumento volumétrico da glândula tireoide. Quando esse aumento ocorre de forma a comprimir a luz traqueal, pode causar sintomas respiratórios ao paciente, havendo indicação de tratamento cirúrgico. O procedimento pode ter complicações catastróficas no intraoperatório se não for realizado o controle adequado da via aérea do paciente, havendo a necessidade de intubação com ventilação espontânea. O caso relatado a seguir ilustra a abordagem cirúrgica de um bócio volumoso e as possíveis complicações deste procedimento.
