En este artículo se presentan una visión panorámica de la Atención Primaria en Salud (APS) en Paraguay, parte de un estudio multicéntrico para identificar las posibilidades de la APS como reordenador del sistema. Adopta el concepto de APS abarcadora y lleva en consideración la segmentación del sistema, conformado por: el sector público del Ministerio de Salud Pública y Bienestar Social, el Seguro Social (Instituto de Previsión Social), el sector privado sin fines de lucro, privado con fines de lucro y mixto. Analiza cinco dimensiones: rectoría, financiamiento, recursos, integración/continuidad, intersectorialidad/participación, por medio de la revisión de literatura, documentos oficiales, y entrevistas con actores claves (expertos, gestores, profesionales y sociedad civil). Visualiza avances en el marco jurídico sanitario desde los años 1990, orientado a la gratuidad en todos los grupos de edad, concretizada en 2008. El gasto público en salud está entre los más bajos de América Latina. La APS se desarrolla en diferentes programas verticales, con escasa coordinación y articulación, pero un cambio reciente en la política prioriza la instalación progresiva de equipos de Salud Familiar. En conclusión, la APS puede contribuir para aumentar la salud, equidad y participación.
This study analyzes the conduction patterns of implementing Primary Health Care (PHC) in Paraguay in three government periods (2008-2012, 2012-2013 and 2013-2017) and three management levels (national, regional and local). This is a qualitative study based on grounded theory. A priori categories on PHC stewardship and governance in Paraguay were analyzed. An open-ended questionnaire was applied to a sample of social, political and technical stakeholders: ministers, coordinators, managers, consultants, and international organizations' experts. Data were processed combining the use of Atlas Ti software and sorting findings in a structured Excel matrix. Gaps in leadership, regulatory mechanisms, technical capacities for health planning and management and financial implementation methods have affected PHC continuous expansion and strengthening process. The findings show limitations and possibilities for the implementation of this health policy in Paraguay, evidencing the need for greater qualification of management and political stability in its conduction.
En Paraguay, se dispone de escasa información sobre cómo se organizan y controlan las condiciones higiénico sanitarias de servicios de alimentación de hospitales públicos y privados. Diferentes problemas de carácter estructural y funcional han sido identificados desde diversas instancias en los servicios de alimentación de establecimientos de salud públicos y privados. Los servicios de alimentación y nutrición de los hospitales tienen como misión dar respuesta a la necesidad de alimentación de los usuarios con alimentos en óptimas condiciones higiénicas y organolépticas en los tiempos establecidos.

Objective:To analyze the causes of the legal void to which the delay in the execution of the Primary Health Care strategy in Paraguay is attributed in the first two decades following the Alma Ata Declaration (1978), based on the review of the historical context, regulatory framework and expert opinion. Methodology: This is a descriptive, observational, retrospective historical, qualitative study, based on the review of documentation (the APS regulatory framework from 1978 to 2008) and primary sources gleaned from key informant interviews from a sample of restricted actors chosen for convenience (former ministers, former vice ministers, managers and experts), establishing the legal scope as categories of analysis. Results: Between 1978 and 1990, resolutions without legal effect have backed temporary projects for the development of a selective and partial APS strategy model, not yet interpreted as an entryway to the Health System, confirming the absence of laws or decrees that effectively permitted it. Conclusion: Only when a written norm is followed by the weight of the law it has been able to establish the relationship between the legal framework, the understanding of the conceptual definition of the APS strategy, as an entryway to the Health System in Paraguay, and its effective implementation as a state policy.
